Job Description Summary

The Licensed Independent Social Worker (LISW) – AYA Cancer Survivorship is an embedded member of the Survivorship Program, providing licensed psychosocial assessment, navigation, and therapeutic support to cancer survivors. The role is anchored in Adolescent and Young Adult (AYA) survivorship, with additional capacity to support the broader survivorship population as program needs and capacity allow. This approach ensures focused expertise for AYA survivors while strengthening psychosocial support across survivorship services.

The LISW works in close collaboration with interdisciplinary survivorship teams to deliver coordinated, patient‑centered care. The role provides psychosocial assessment and therapeutic interventions that require licensed behavioral health expertise, complementing—but not duplicating—clinical coordination and survivorship care planning functions.

Job Description

The Licensed Independent Social Worker (LISW) – AYA Cancer Survivorship is an embedded member of the Survivorship Program, providing licensed psychosocial assessment, navigation, and therapeutic support to cancer survivors. The role is anchored in Adolescent and Young Adult (AYA) survivorship, with additional capacity to support the broader survivorship population as program needs and capacity allow. This approach ensures focused expertise for AYA survivors while strengthening psychosocial support across survivorship services.

The LISW works in close collaboration with interdisciplinary survivorship teams to deliver coordinated, patient‑centered care. The role provides psychosocial assessment and therapeutic interventions that require licensed behavioral health expertise, complementing—but not duplicating—clinical coordination and survivorship care planning functions.

Job Responsibilities Include:

(1) Clinical & Psychosocial Support - AYA Survivorship (25%)

Provides AYA‑focused psychosocial assessment, navigation, and licensed therapeutic interventions for cancer survivors transitioning from treatment to survivorship. Delivers developmentally appropriate counseling addressing: (1) anxiety, depression, and psychosocial distress, (2) fertility and family‑planning concerns, (3) education, employment, and financial stressors, (4) identity development, relationships, and social reintegration. Provides longitudinal psychosocial support within the AYA Survivorship Program.

(2) Clinical & Psychosocial Care - General Survivorship (20%)

Provides psychosocial assessment, counseling, and therapeutic support to survivors within the general survivorship population. Supports patients experiencing adjustment challenges, psychosocial distress, or barriers to care during long‑term survivorship and follow‑up.

(3) Program Development & Support (20%)

Supports development and implementation of the AYA Survivorship Program, including: (1) psychosocial screening workflows, (2) referral pathways, (3) patient engagement strategies. Participates in survivorship clinics, interdisciplinary meetings, program development initiatives, and community outreach engagements.

(4) Care Coordination & Collaboration (20%)

Collaborates closely with the Survivorship Program Team and the Survivorship Nurse Navigator to ensure coordinated, holistic survivorship care while maintaining clear role delineation. Coordinates internal referrals to behavioral health, fertility services, rehabilitation, wellness, and other supportive care programs. Serves as an embedded psychosocial resource within survivorship clinics to reduce reliance on external referrals and minimize delays in care.

(5) Data Tracking, Quality Improvement & Reporting (10%)

Maintains accurate documentation of clinical encounters, referrals, and outcomes. Supports data collection for survivorship metrics, quality improvement initiatives, patient experience efforts, and program evaluation. Identifies gaps in survivorship care delivery and contributes to improvement strategies.

(6) Professional Development & Institutional Engagement (5%)

Engages in ongoing professional education related to survivorship, AYA oncology, and evidence‑based psychosocial practices. Participates in interdisciplinary meetings, training activities, and institutional efforts that enhance the quality and visibility of survivorship services.

Additional Job Description

Education:  Master's or Doctorate degree in social work from an accredited program.  Work Experience:  4-6 years

Licensure: Current SC LISW license
