- Current Texas Licensed Practical Nurse license.
- Three years of clinical nursing experience.
- Current Texas driver's license.
- Current Basic Life Support certification.
- Certification in cardiopulmonary resuscitation is required.
- Experience with healthcare software.
- Competency to perform skilled procedures ordered by a physician within the scope of the Nurse Practice Act.
- Ability to treat wounds.
- Strong organizational and time-management skills.
- Ability to multitask and meet timelines.
- Professional appearance and effective communication skills.
- Provide comprehensive care in patients' homes, including completing skilled documentation through Homecare Homebase.
- Serve as the patient's advocate and maintain ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
- Administer medications and treatments as prescribed by physicians.
- Provide direct patient care, including infusion therapy, wound care, teaching, and disease management.
- Participate in regular case conferences and in-service training.
- Serve approximately six patients per day from Monday through Friday.
- Previous home health experience.
