Requirements
  • A New York State nursing license is required.
  • Specialized training in intellectual and developmental disabilities and/or one year of experience in community health care services is required.
  • Strong verbal and written communication, organizational, detail-oriented, interpersonal, and team-building skills are required.
  • Computer skills and knowledge of word processing are required.
  • The ability to work effectively with people of diverse races, ethnicities, ages, and sexual orientations in a multicultural environment is required.
  • CPR and First Aid training are required.
  • The ability to set limits, maintain role, and work collaboratively and respectfully is required.
Responsibilities
  • Coordinate all medical and psychiatric care and medications for residents, ensuring proper documentation, follow-up, and regulatory compliance.
  • Assist with hands-on training of residential staff to maintain compliance.
  • Identify and organize appointments in conjunction with the registered nurse and manager.
  • Transport and accompany residents to scheduled medical appointments and complete necessary follow-up.
  • Perform delegated nursing tasks as directed by and under the supervision of the registered nurse.
  • Coordinate medication administration, including verifying prescriptions, ordering medications, completing regulatory documentation, and coordinating staff administration with the registered nurse.
  • Maintain inventory of prescription medications, over-the-counter medications, and first aid supplies, including routine expiration-date checks.
  • Maintain medically related documentation, including medication administration records and physician consultations.
  • Update the registered nurse daily on residents' medical status.
  • Complete documentation before the end of each shift and report issues and concerns to the registered nurse and residence manager.
  • Complete occurrence reports as required.
  • Review and make daily entries in the house log book regarding pertinent resident and residence issues.
  • Monitor resident progress and notify applicable parties of concerns, behavioral changes, or progress.
  • Implement directives issued by the registered nurse and manager.
  • Handle telephone calls and inquiries, provide follow-up, and keep family and management informed when necessary.
  • Apply emergency procedures, including fire safety procedures, during crises.
  • Act as liaison between the residence, day program, physicians, and families regarding residents' medical issues.
  • Provide residents with instruction or assistance in daily living, rehabilitation, or social skills according to the Individual Service Plan.
  • Provide ongoing training to residents and staff in areas identified in the Individual Service Plan, in conjunction with the registered nurse and manager.
  • Provide client-specific hands-on training to staff as directed by the registered nurse.
  • Attend annual and semi-annual reviews and provide medical input when the registered nurse is not involved.
  • Visit residents in the hospital as needed and coordinate discharge plans with the registered nurse.
  • Complete residential habilitation notes, progress notes, and other required documentation.
  • Comply with Occupational Safety and Health Administration regulations, universal precautions, AMAP training, annual recertification training, and annual PPD clearance requirements.
  • Attend meetings and in-services and participate in staff meetings and mandatory annual in-services as directed by the residence manager and registered nurse.
  • Supervise residents' intake of prescribed medication in accordance with the OMRDD-approved Medication Administration Procedure.
