Requirements
- Be licensed as a Nurse Practitioner or Physician Assistant in New York or New Jersey, or be willing to obtain the applicable license.
- Have prior experience in skilled nursing, internal medicine, or endocrinology; this experience is preferred.
- Communicate effectively and use a patient-centered approach.
- Work independently and collaboratively.
Responsibilities
- Perform on-site patient evaluations and follow-ups.
- Manage diabetes, thyroid disorders, and other common endocrine conditions.
- Collaborate with Directors of Nursing, facility leadership, and primary care teams.
- Document care in facility electronic medical records.
- Support initiatives that reduce unnecessary hospitalizations and improve hemoglobin A1C and finger-stick outcomes.
