Requirements
  • A Master of Social Work (M.S.W.) or another appropriate master's-level degree, such as an MPH or MFT, with additional or specialized work experience in areas such as psychology, counseling, or geriatrics.
  • Alternatively, a bachelor's degree in Social Work or another appropriate major, at least two years of relevant social work experience, and the ability to independently conceptualize and formulate biopsychosocial assessments, identify care needs and necessary interventions, and execute effective care interventions.
  • At least one year of experience working with disabled adults and/or older adults.
  • Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
  • Exceptional communication and presentation skills when working with functionally impaired adults and older adults, their support systems, and teams of health professionals.
  • Demonstrated case management skills and experience in the community health care delivery system.
  • Computer literacy.
  • Ability to apply the ethical and legal standards of the National Association of Social Workers and the Institute on Aging's regulatory and procedural requirements.
Responsibilities
  • Conduct comprehensive assessments and ongoing reassessments of clients' psychosocial, physical and mental health, environmental, and spiritual needs.
  • Write comprehensive assessments and develop and initiate a client-centered Community Living Plan with the client, consistent with program guidelines, policies, and procedures.
  • Conduct home visits and visits to acute hospitals and skilled nursing facilities, and escort clients to medical and other appointments as clinically indicated.
  • Identify, arrange, and monitor appropriate community services based on knowledge of Medicare, Medi-Cal, and other entitlement programs.
  • Establish and maintain care management relationships with clients and their informal support networks, providing crisis intervention, advocacy, problem solving, and therapeutic interventions while offering respect, dignity, and support.
  • Meet with clients at least monthly, or more often as clinically indicated, and review and modify their Community Living Plans on an ongoing basis.
  • Document case management activity relating to identified problems through progress notes within 24–48 hours, adding new problems to the Community Living Plan as needed.
  • Maintain required paperwork and a clear, concise, consistent charting system to support continuity of care.
  • Evaluate ongoing client Purchase of Service needs and follow up to determine whether services have been provided in a timely manner.
  • Educate clients and their informal support networks about available resources.
  • Maintain effective communication with community providers, including physicians and other health care and social service workers, sharing significant information about client care and program operations while maintaining confidentiality.
  • Monitor the quantity and quality of services provided by other involved providers.
  • Work with the team to evaluate clients' ability to remain safely at home and coordinate placement as appropriate under program guidelines.
  • Work with the client, caregiver, and involved services to terminate services when appropriate, and document the process as required.
  • Participate in research studies and data collection as required.
  • Participate in and promote Continuous Quality Improvement efforts.
  • Attend and actively participate in team and program meetings, activities, and problem-solving efforts, contributing to communication within the team.
  • Use supervision appropriately, maintain open communication, and provide updates on caseload activity.
  • Attend continuing education classes and/or in-service training to develop knowledge, skills, and attitudes related to case management, gerontology, family and community systems, and other areas relevant to the Community Living Fund client population.
  • Perform other reasonably related responsibilities as assigned.
