About the job
- A Doctor of Pharmacy degree from an accredited institution.
- Licensure as a pharmacist in the state of practice.
- Three years of experience in a pharmacy setting.
- Proven experience as a clinical pharmacist.
- Excellent understanding of pharmaceutical therapy and direct patient care.
- Ability to manage multiple priorities in an expedient and decisive manner.
- Proficiency with Google Suite, including Drive, Docs, Sheets, and Slides, and Microsoft Office, including Word, Excel, and PowerPoint, for real-time collaboration.
- Ability to stand, walk, push, bend, kneel, and reach.
- Ability to sit for extended periods of time.
- Corrected vision and hearing within the normal range.
- Ability to operate a motor vehicle and have personal transportation.
- Ability to handle data confidentially.
- Ability to work cross-functionally with multiple teams and independently with minimal supervision.
- Strong organizational, time-management, multitasking, written and verbal communication, interpersonal, presentation, critical-thinking, and problem-solving skills.
- Comfort communicating with senior-level leadership, providers, and health plans.
- Appreciation of cultural diversity and sensitivity toward target patient populations.
- Develop and maintain protocols for prescribing, dispensing, and administering medications.
- Conduct research to develop or improve pharmaceutical treatments.
- Provide specialized services to help patients manage conditions such as diabetes, chronic obstructive pulmonary disease, congestive heart failure, asthma, and hypertension.
- Participate in quality improvement initiatives to contribute to efficient care delivery.
- Participate in the development or revision of pharmacy policies and procedures.
- Provide training and support to pharmacy staff and interns.
- Maintain current knowledge of new drugs, medical devices, and available therapies.
- Ensure compliance with federal, state, and local pharmaceutical regulations.
- Consult with physicians regarding drug-prescribing practices.
- Participate in patient intervention and referral, risk management, and ethical decision-making.
- Participate in multidisciplinary team meetings and contribute to overall patient care plans.
- Collaborate with healthcare professionals to plan, monitor, review, and evaluate patient effectiveness.
- Educate patients about medications, side effects, interactions, and therapeutic effects.
- Review patient profiles to ensure medication safety and efficacy.
- Participate in patient-care rounds and perform drug-therapy consultations as required.
- Perform additional duties as assigned.
