Requirements
  • Pass the Department of Human Services background check and maintain qualified status.
  • Not be excluded from working in government health programs by the Office of Inspector General.
  • Successfully meet the company’s physical job demands.
  • Have a valid current driver’s license and vehicle insurance as required by state law.
  • Successfully complete the Medication Administration class with a score of 80% or higher and regularly pass medications using the correct procedure without error.
Responsibilities
  • Assist clients with daily care, medical needs, activities of daily living, and mental health supports.
  • Assist individuals with daily routines, including hygiene, domestic tasks, leisure activities, grooming, socializing, budgeting, money usage, safety and health, studying, homework completion when applicable, and community participation.
  • Plan and implement recreation, leisure, and community activities.
  • Drive clients to and from appointments and community activities and accompany them using a company vehicle during all hours of operation.
  • Assist individuals in establishing and maintaining community relationships and facilitating friendships.
  • Implement programs in accordance with the Coordinated Service and Support Plan, Coordinated Service and Support Plan Addendum, and Individual Abuse Prevention Plan.
  • Provide person-centered services through practice, action, and language.
  • Maintain knowledge of clients’ histories, skills, vulnerabilities, habilitation plans, and emergency procedures.
  • Participate in the support team process and assist with developing and implementing goals.
  • Administer and document prescribed medications, treatments, diets, and exams as directed by the Coordinated Service and Support Plan and Addendum.
  • Maintain confidentiality in accordance with HIPAA and MORA policies and procedures.
  • Prepare meals according to the planned menu and complete weekly food ordering as needed or scheduled.
  • Assist with daily housekeeping and household maintenance.
  • Complete house quality-improvement assessments and address identified issues.
  • Ensure adequate client supervision and remain on shift until a replacement employee arrives.
  • Communicate verbally and in writing using the primary spoken language of clients in the home.
  • Complete mandatory training required by MORA or Minnesota 245D regulations.
  • Read and follow company policies and procedures.
  • Remain awake at all times unless working an assigned sleeping shift.
  • Document program data and daily log information accurately and promptly.
  • Collaborate with the team and complete other assigned duties.
  • Schedule and attend client appointments and prepare referrals and other required documents for clinicians or external parties.
  • Oversee the exchange of medical appointment information, document and relay updates or concerns to the interdisciplinary team, MORA nurse, and house team.
  • Oversee the medication administration record, keeping medication lists and required client information current.
  • Order medications, prepare the new monthly medication administration record, file old records, check medications weekly, and remove expired medications.
  • Ensure the safe use of clients’ adaptive medical equipment.
  • Attend medical specialist training sessions and meetings.
  • Use flexible hours for client needs such as annual meetings, medical appointments, and day-program closures.
  • Use flexible hours on-site to oversee administrative home tasks, including assisting with training, house scheduling, employee onboarding, employee performance evaluations, client care, house audits, and licensing activities.
Desired Qualifications
  • Interest and/or experience working with persons with intellectual and developmental disabilities in a healthcare oversight capacity.
  • Three to five years of experience with clients with intellectual and developmental disabilities and medical coordination needs.
  • Proven leadership skills.
  • Ability to work with and support a team in understanding client healthcare needs, managing appointments, and working closely with the interdisciplinary team and MORA team.
