  • Graduation from an accredited school of Licensed Practical Nursing is required.
  • New Hampshire Licensed Practical Nurse licensure is required.
  • Current Basic Life Support certification is required, or it must be obtained within 90 days of hire.
  • At least two years of nursing experience is required.
  • The ability to perform nursing triage is required.
  • A valid driver's license is required for travel throughout southern New Hampshire.
  • Proof of automobile insurance is required.
  • A motor vehicle background report will be conducted.
Responsibilities
  • Conduct timely post-discharge telephone follow-up with patients to assess clinical status, reinforce discharge instructions, and identify potential risks or unmet needs.
  • Address patient questions, concerns, and barriers to care to promote patient satisfaction.
  • Identify trends or gaps affecting patient satisfaction and quality outcomes and communicate findings to leadership.
  • Monitor and accurately document patient interactions in the electronic medical record according to agency policy and regulatory standards.
  • Serve as a resource to patients and caregivers to ensure continuity and clarity of care after discharge from home health services.
  • Support Value-Based Purchasing and Quality Assurance and Performance Improvement initiatives, including patient experience, care coordination, and outcome improvement.
  • Assist with assigned quality audits.
  • Assess patient-reported symptoms and conditions by telephone and apply clinical judgment to triage needs appropriately.
  • Coordinate with clinical managers to escalate concerns, initiate interventions, or adjust care plans based on patient status.
  • Collaborate with scheduling staff to prioritize and facilitate timely planned and as-needed visits based on patient acuity and needs.
  • Help prevent avoidable hospital readmissions by identifying clinical deterioration early and promptly escalating concerns through established clinical or disease-specific protocols and notifying the provider.
  • Reinforce patient and caregiver education about disease management, medications, and when to seek additional care.
