Requirements
  • A high school diploma or GED is required.
  • At least 3 years of patient support engagement or customer service experience in a healthcare environment working directly with patients is required.
  • Demonstrated sales aptitude and the ability to navigate conversations with new patients learning about Evergreen's care model are required.
  • Experience managing workload to achieve defined metrics or targets is required.
  • Experience in a team-based service environment is required.
  • Genuine curiosity when engaging with patients and conversational intelligence are required.
  • Excellent verbal and written communication skills are required.
  • Intermediate skills with Microsoft Office Suite products, including Outlook and Teams, are required.
  • Ability to work effectively in a primarily remote environment is required.
  • Home internet must support at least 25 Mbps download and 10 Mbps upload speeds.
  • Employees must work from a dedicated space without ongoing interruptions to protect member protected health information and HIPAA information.
Responsibilities
  • Manage a high volume of outbound and inbound new patient onboarding calls.
  • Build trust with patients and clinical care team members through omnichannel customer service.
  • Listen during patient conversations to activate personalized support for new patients and provide appropriate next steps, including visit scheduling.
  • Collaborate with interdisciplinary care team members, including nephrologists, practice staff, nurse care managers, advanced practice providers, social workers, and dieticians.
  • Assist with patient and practice administrative requests, including new patient onboarding outreach, connection to local care teams, scheduling assistance, and intake of patient complaints.
  • Help build workflows, processes, and documentation related to contact center operations.
  • Execute engagement initiatives supporting strategic goals related to Clinical Excellence, Clinical Quality, and Patient Engagement.
  • Perform other duties consistent with the role as assigned.
Desired Qualifications
  • A bachelor's degree is preferred.
  • Startup experience is a plus.
  • Experience with Medicare and/or kidney care populations is a plus.
  • Bilingual Spanish and English fluency is a plus.
