Requirements
  • A Bachelor's degree in nursing or health care is preferred.
  • A current Registered Nurse license to practice as a professional nurse in the state where the office is located is required.
  • At least 3 years of nursing experience is required, including 1 year of full-time direct patient care experience in hospice, home health, or oncology.
  • Current CPR certification must be maintained if required by the state.
  • A current driver's license, vehicle insurance, and access to dependable transportation are required.
  • The candidate must be able to manage all aspects of terminal care, including crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.
  • The candidate must demonstrate a supportive and sensitive approach to staff and patient/family needs.
  • Computer proficiency, including typing, use of various programs, and electronic medical record systems, is required.
Responsibilities
  • Coordinate and oversee patient services in alignment with established hospice program policies.
  • Provide guidance and support to the clinical team to continually improve hospice care services.
  • Conduct chart audits and oversee medical record upkeep for regulatory, certification, and ethical-care compliance.
  • Help create orientation and in-service training for nursing and clinical staff.
  • Evaluate clinical staff performance during probationary and annual reviews, and provide teaching and counseling to improve service delivery.
  • Lead interdisciplinary team meetings to refine patient and family care plans, exchange insights, and resolve challenges.
Desired Qualifications
  • One year of supervisory experience in hospice or home health care is preferred.
