  • Valid driver’s license and auto liability insurance
  • Bilingual – English/Spanish

  • Identify health/wellness goals and incorporate into Health Action Plan
  • Collaborate with care management team
  • Meet monthly encounter metrics
  • Break down barriers for individuals
  • Assist in securing connection to community supports
  • Coordinate physical care management appointments
  • Utilize online platforms for collaboration
