  • Master’s degree in substance use and/or mental health field (psychology, social work, counseling) required
  • Minimum of 1+ years of SUD clinical care experience.
  • Experience with motivational Interviewing, and Triage.
  • Excellent listening, written and verbal communication skills.
  • Acknowledge and embrace diversity and inclusion.
  • Experience working with diverse populations.
  • Motivated to go above and beyond for members to provide a high-touch experience.
  • Tech-savvy: must be comfortable using various computer platforms and learning new systems, and efficient in tech-related tasks.
  • Able to multi-task and context switch!
  • KPIs will include: metrics on ticket resolution, connection to care, care conversion, and member satisfaction.
Responsibilities
  • Conduct outbound calls to individuals who have expressed interest in substance use disorder treatment or who may benefit from our services.
  • Engage with clients in a compassionate and empathetic manner to assess their needs and provide information about available treatment options.
  • Provide information about our treatment programs, services, and available resources to potential clients.
  • Coordinate appointments, referrals, and follow-up care to ensure continuity and quality of care.
  • Apply active listening, psycho-education, and motivational interviewing skills to understand each member’s needs and provide support as they set goals to decrease substance use and/or to achieve successful recovery.
  • Familiar with substance use disorders, ASAM levels of care, and SUD treatments such as contingency management, MAT, detox.
  • Provide consultation regarding mental health concerns and family/relationship issues, as appropriate to member needs.
  • As necessary, provide in-the-moment support and safety planning for members with safety, risks or crisis.
  • Utilize written communication skills to interact with members and providers through email.
  • Document information, maintain accurate records and track progress within several systems, including an EHR within required timeframes.
  • Identify and problem-solve issues that serve as a barrier or disruption to care.
  • Interact with multi-disciplinary teams such as care support, provider operations, and benefits/billing to ensure a smooth member experience.
  • Participate in staff meetings, case consultations, and training.
  • Follow Spring Health policies and maintain all confidentiality, compliance, and ethical standards.
  • Provide support and coverage to other SUD CNs & Advocates on the team.
