Requirements

  • An associate or bachelor's degree in nursing.
  • One year of clinical registered nurse experience in home health or hospice.
  • One year of registered nurse management experience.
  • A current registered nurse license specific to the assigned work state(s).
  • Current cardiopulmonary resuscitation certification.

Responsibilities

  • Support, participate, and work cooperatively with the director of staffing.
  • Be responsible for overall quality of patient care and the clinical performance of nurses and aides.
  • Organize clinical operations for the care center.
  • Review requests for services and determine patient eligibility and suitability for home care services; coordinate patient referrals and admissions with intake.
  • Oversee scheduling.
  • Oversee payor verification and precertification requirements.
  • Review documentation of other staff members and ensure missing, incomplete, or untimely documentation issues are resolved.
  • Assist clinicians in establishing immediate and long-term patient goals, setting priorities, and developing plans of care.
  • Regularly evaluate patient needs and make necessary revisions to plans of care in collaboration with clinicians.
  • Provide direct patient care infrequently in emergencies, as needed.
  • Participate in the on-call process.
  • Assist with staff training to ensure quality care, compliance, and fiscal responsibility.
  • Participate in investigating and resolving patient, family, and physician concerns.
