The Role

GHMG is currently seeking contract, Telehealth Social Workers to join our remote team. We’re looking for licensed professionals who bring deep empathy for vulnerable patients and their families, and who provide care that is compassionate, thorough, and patient-focused. Strong communication skills and comfort with technology are essential.

What You’ll Do

• Provide supportive counseling, psychosocial assessments, and care planning for patients and families navigating serious illness and end-of-life care.

• Collaborate with the interdisciplinary care team to ensure coordinated, comprehensive care.

• Follow best practices and company-specific policies for documentation, treatment planning, and communication.

• Ideally 10 hours per week available for patient care activities.

What You’ll Bring

• Fully licensed (LCSW, LPC, LMHC) in any USA state

• Minimum 2 years of post-licensure clinical experience, ideally in palliative care, hospice, serious illness, or related settings.

• Experience providing care through telehealth platforms preferred.

• Compassionate communication skills and the ability to support patients and families during challenging care journeys.
