Requirements
  • Must pass the Department of Human Services background check and maintain qualified status.
  • Must not be excluded from working in government health programs by the Office of Inspector General.
  • Must successfully meet the company’s physical job demands.
  • If using a personal vehicle for company business, must have a current valid driver’s license and insurance required by state law.
  • Must successfully complete the Medication Administration class with a score of 80% or greater and regularly pass medications using the correct procedure without error.
  • Must be able to use a computer in alignment with client needs and company procedures.
Responsibilities
  • Assist clients with activities of daily living, personal care, medical needs, supports, and mental health supports.
  • Assist individuals with daily routines, including hygiene, domestic tasks, leisure, grooming, socializing, budgeting, safety and health, studying, homework when applicable, and community participation.
  • Plan and implement recreation, leisure, and community activities.
  • Drive to and from, and accompany clients to, appointments and community activities using a company vehicle during operating hours.
  • Assist individuals in establishing and maintaining community relationships and facilitating friendships.
  • Implement programs in accordance with the Coordinated Service and Support Plan, its Addendum, and the Individual Abuse Prevention Plan.
  • Provide person-centered services through practice, action, and language.
  • Maintain working knowledge of each person’s history, skills, vulnerabilities, habilitation plans, and emergency procedures.
  • Participate in the support team process and assist with developing and implementing goals.
  • Administer and document prescribed medications, treatments, diets, and examinations as directed by the service plans.
  • Maintain confidentiality in accordance with HIPAA and company policies and procedures.
  • Prepare meals according to the planned menu and complete weekly food ordering as needed or scheduled.
  • Assist with daily housekeeping and household maintenance.
  • Complete house quality-improvement assessments and address identified issues.
  • Ensure adequate client supervision, including remaining on shift until a replacement arrives.
  • Communicate effectively verbally and in writing, using the primary spoken language of clients in the home.
  • Complete mandatory training required by the employer or applicable 245D regulations.
  • Read and follow company policies and procedures.
  • Remain awake unless working an assigned sleeping shift.
  • Document program data and daily log information accurately and promptly.
  • Collaborate with the team and complete other assigned or needed duties.
  • Train new staff on client mental health outcomes, positive support strategies, behavior programming, and information specific to client mental health diagnoses.
  • Work with the Program Supervisor and Positive Supports Manager to create and update mental health outcomes.
  • Complete training on specific mental health diagnoses for supported clients.
  • Work with the Program Coordinator to ensure staff log data on behavioral and mental health programming.
Desired Qualifications
  • Interest in or experience working with people with intellectual and developmental disabilities in a mental health capacity.
  • Three to five years of experience with clients who have intellectual and developmental disabilities and mental health needs.
  • Three to five years of experience in positive supports and mental health outcomes.
  • Proven leadership skills.
  • Ability to support a team in understanding client mental health needs and work closely with the interdisciplinary team and employer team.
