Requirements
  • A Master of Social Work from a Council on Social Work Education-accredited program is required.
  • Current licensure as a Licensed Clinical Social Worker in the State of Texas is required.
  • A criminal history background check is required for finalists.
  • Candidates must present acceptable original documents proving identity and authorization to work in the United States by the third day of employment.
Responsibilities
  • Conduct specialized psychosocial assessments for patients in the cardiac and kidney transplant programs to help the transplant team evaluate psychological and social readiness for surgery, recovery, and rehabilitation.
  • Document interactions with patients, families, and service providers accurately and promptly in the clinical record while adhering to professional ethics and federal and state regulations.
  • Provide therapeutic recommendations and clinical interventions, and collaborate with patients and families to develop individualized treatment plans.
  • Provide consultation, orientation, and training to healthcare staff regarding biopsychosocial, cultural, and economic factors affecting patient care.
  • Collaborate with the interdisciplinary team on care coordination and complex discharge planning.
  • Help patients access housing, transportation, immigration support, disability services, medical insurance, and other financial resources.
  • Participate in transplant team care conferences and selection committee meetings.
  • Contribute to program policies, procedures, and social work activities related to transplantation.
  • Support Centers for Medicare & Medicaid Services and United Network for Organ Sharing survey-readiness efforts.
  • Participate in transplant quality assurance and performance improvement initiatives.
  • Participate in research initiatives intended to improve patient care operations.
  • Screen, track, and report patient outcomes and social work interventions.
  • Participate in Health Social Work meetings, continuing education, academic opportunities, and research opportunities.
  • Contribute mental health, cultural competency, and social determinants of health expertise to the interdisciplinary team.
Desired Qualifications
  • Three years of post-degree clinical experience in a healthcare setting.
  • A Certified Clinical Transplant Social Worker credential and transplant experience.
  • Experience conducting assessments and delivering evidence-based treatments, with working knowledge of brief interventions, motivational interviewing, and healthcare delivery.
  • Experience working with children, adolescents, and their families.
  • Bilingual proficiency in English and Spanish.
  • Basic Life Support certification.
