- Successful completion of an accredited Licensed Practical Nursing program and 6–12 months of healthcare experience are required.
- A Licensed Practical Nurse license in good standing with the State of Michigan is required.
- Basic Life Support certification is required.

- Must be able to sustain attention and concentration during computer-based and detailed work, stand or walk regularly, lift light objects up to 20 pounds, and assist with lifting and moving patients.
- Must be familiar with insurance and referral policies.
- Work under a provider’s direction to perform general nursing staff functions for the practice.
- Record vital signs and immunizations, perform initial patient assessments, obtain concise patient histories, and provide clinical information and support to providers.
- Process and post information to patient charts and educate patients and families about medications, treatment, care, and referrals.
- Prepare daily patient charts, maintain documentation standards, and review charts before appointments to ensure required requests and documents are available.
- Prepare patients for examinations by performing assessments, taking histories and vital signs, reconciling medications, conducting routine hearing and vision testing, and providing initial assessments to physicians.
- Triage calls to determine urgency, the need for an appointment, or whether telephone advice is sufficient.
- Assist providers with minor and emergency procedures and examinations, follow physician instructions, and reassure patients during examinations.
- Schedule tests and procedures in other departments or facilities under provider direction and prepare patients by providing appropriate instructions.
- Perform laboratory tests as directed by physicians and accurately document the required information in patient charts.
- Coordinate admissions, referrals, prescription calls, and related activities.
- Provide patients and families with appropriate education regarding their care under provider direction.
- Take verbal, telephone, and written medication orders from physicians, dispense ordered medications as directed, and properly record all data.
- Maintain and review patient records, charts, and other pertinent information, and record test assessment results.
- Participate in performance-improvement monitoring, collect and collate quality-of-care data, and help ensure delivered care meets appropriate standards.
- Communicate with referring providers and forward records as requested by patients.
- Assist with completing disability, workers’ compensation, and State of Michigan forms.
- Attend staff, educational, and other required meetings.
- Perform other related duties as assigned.
