Requirements
Hold a license and current registration to practice as a Registered Professional Nurse or Licensed Practical Nurse in New York State.
Have at least 2 years of experience as an RN in home healthcare or a Medical/Surgical program, such as critical care, emergency room, or wound care.
Be able to assess and respond to the needs of patients and families in the home setting.
Maintain professional liability insurance in the amount specified by VNSW.
Be able to use industry technology such as a tablet while working in the field with patients.
Have a working automobile available for field use and be able to drive.
Responsibilities
Ensure the quality and safe delivery of home health services.
Assess, plan, implement, evaluate, and coordinate care in the home in collaboration with the patient.
Supervise and train Home Health Aides.

Fee For Service: $50 to $125 per visit, depending on the type of visit.
Our VNSW@Home nurses ensure the quality and safe delivery of home health services. They assess, plan, implement, evaluate, and coordinate all care in the home. The nurse carries out this process in a collaborative relationship with the patient and through the supervision and training of Home Health Aides.
Qualifications: License and current registration to practice as a Registered Professional Nurse or Licensed Practical Nurse in the State of New York.
Minimum of 2 years of experience as an RN in a home healthcare or Medical/Surgical program such as critical care, ER, Wound care and similar types of specialties.
Ability to assess and respond to the needs of patients and families in the home setting.
Professional liability insurance in the amount specified by VNSW.
Ability to utilize industry technology such as a tablet when in the field with patients.
Ability to drive with automobile in good working condition and available for use in the field.
