About the job

Requirements
  • Real home health chart review experience, including familiarity with comprehensive assessments, physician notes, and therapy records, and the ability to identify discrepancies.
  • Active or in-progress OASIS certification, such as HCS-O, COS-C, or another company-approved credential; an in-progress certification must be pursued on a defined timeline.
  • A current, applicable ICD-10-CM coding credential, such as HCS-D, BCHH-C, CPC, or RHIT.
  • Ability to work in an AI-assisted workflow, critically reviewing model output while recognizing accurate suggestions.
  • Ability to write clear, concise QA comments that do not require follow-up clarification.
  • Ability to escalate rather than guess when a chart does not clearly support an answer.
  • Maintain HIPAA compliance and confidentiality for patient, client, and AI workflow data.
  • Maintain required certifications and promptly notify management if a certification lapses.
  • OASIS certification may be required at hire or obtained within a management-approved timeframe; a current applicable coding credential is required according to company standards and client scope.
Responsibilities
  • Complete assigned Home Health QA reviews accurately and on time, following company standards, client expectations, ICD-10-CM coding guidelines, OASIS guidance, payer requirements, AI-supported workflow expectations, and internal quality protocols.
  • Manage the assigned review queue and quota, prioritize AI routing queues, holds, and urgent items, meet turnaround times, and promptly flag delays or access issues.
  • Validate ICD-10-CM accuracy, sequencing, specificity, and guideline compliance, including diagnoses, symptom and manifestation codes, etiology/manifestation pairing, laterality, encounter type, and documentation support.
  • Confirm OASIS responses are clinically supported, internally consistent, and aligned with CMS guidance by cross-referencing the comprehensive assessment, referral, physician documentation, therapy notes, wound care, medications, and hospital records; do not accept copy-forward answers.
  • Prioritize review of GG items and functional status, M1800 ADLs, cognition, pain, dyspnea, falls, wounds, medication management, hospitalization risk, and discharge disposition.
  • Place charts on hold or escalate them when documentation is incomplete, unsigned, unclear, or contradictory, and write actionable QA comments.
  • Use clinical judgment to review AI-generated coding, OASIS, and documentation suggestions; report recurring errors, missed cues, and workflow gaps through approved channels so corrections can inform training data.
  • Maintain quality scores, respond constructively to secondary-review and audit feedback, and attend training and calibration sessions to support consistent interpretation.

About the company

Nestmed is an enterprise AI software company that automates clinical documentation, medical coding, and quality assurance for home health and hospice agencies by capturing clinician speech during visits and using AI to populate records. Its platform combines an Ambient Scribe that voice-transcribes OASIS forms, care plans, and visit notes; a Coding module that suggests ICD-10 codes from documentation; and a Quality Assurance function that validates OASIS data and flags compliance issues in real time, with each tool sharing patient context so downstream steps arrive pre-populated. What sets Nestmed apart is this integrated approach linking documentation, coding, and QA under shared AI context, built for home health disciplines such as RN, PT, OT, and SLP with field-based alerts rather than delayed reviews. The goal is to reduce clinicians' documentation burden and improve care quality, serving over 17,000 clinicians and documenting more than 7 million visits annually.

What believers are saying

  • Compassus rolled out Nestmed across 300 programs in 33 states on January 26, 2026.
  • VNS Health began enterprise deployment on April 1, 2026 after rigorous pilot testing.
  • Nestmed claims 44% less visit time and 7% higher reimbursement for customers.

What critics are saying

  • EHR vendors like Epic and Microsoft can bundle ambient documentation into existing contracts.
  • Home-care documentation errors trigger compliance and reimbursement clawbacks immediately.
  • If clinician adoption stalls, Nestmed becomes an expensive transcription layer with weak switching costs.

What makes Nestmed unique

  • Purpose-built for home health and hospice, not generic clinic note-taking.
  • Integrates ambient listening, multilingual capture, EMR writeback, QA, and coding workflows.
  • Compassus and VNS Health validated deployment across 33 states and 15,000 clinicians.

VNS Health partners with Nestmed to deploy AI documentation platform across home-based care teams

VNS Health, New York's largest home and community-based healthcare nonprofit, has partnered with Nestmed to deploy an AI-powered clinical documentation platform across its home-based care teams, becoming the first organisation in the state to scale the technology. Nestmed's ambient AI listens during patient visits and automatically generates structured clinical notes, filtering background noise and recognising multiple languages. The notes transfer directly into electronic medical records through a one-click workflow, reducing documentation time for clinicians. The platform addresses administrative burden for VNS Health's clinicians, who served over 61,000 patients last year across diverse New York settings. Nestmed supports over 15,000 clinicians nationwide and powers millions of annual visits. VNS Health completed rigorous evaluation and pilot testing before deployment.
