Requirements
  • Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC).
  • An active Virginia Advanced Practice Registered Nurse license is required; an autonomous practice designation specific to psychiatric practice is preferred.
  • National psychiatric advanced practice registered nurse certification through the American Nurses Credentialing Center or equivalent is required.
  • DEA registration, including the ability to prescribe Schedule II–VI medications and buprenorphine, is required.
  • Active Medicare enrollment via PECOS as an individual provider and active Virginia Medicaid (DMAS) enrollment are strongly preferred.
  • A current, attested CAQH ProView profile is required.
  • No history of Medicare or Medicaid sanctions or exclusions and no unresolved licensure disciplinary actions.
  • Active, unrestricted malpractice and professional liability coverage, or the ability to furnish a certificate of insurance upon offer, is required.
  • Ability to perform evidence-based psychiatric assessments for observation, inpatient admission, or safe discharge decisions, including suicide risk, ability to care for self, and danger-to-others assessments.
  • Ability to work effectively with emergency physicians, hospitalists, surgeons, nursing staff, case managers, and patients.
  • Ability to work independently and build clinical workflows, hospital relationships, and the consult service from the ground up.
Responsibilities
  • Conduct same-day psychiatric consultations for patients referred by emergency, medical, and surgical teams.
  • Collaborate with attending physicians, nursing staff, and case managers to develop and implement treatment plans.
  • Perform rapid assessment, crisis intervention, and stabilization for patients presenting in psychiatric crisis, including suicide and violence risk assessments.
  • Escalate complex management to the collaborating psychiatrist and practice within training and experience with a referral plan for complex cases and emergencies.
  • Provide outpatient stabilization services, medication management, and supportive psychotherapy.
  • Establish and maintain working relationships with hospital departments and community partners.
  • Contribute to consult workflow and care pathway development in collaboration with the consulting psychiatrist.
  • Perform bedside assessments and begin treatment, including medication management, long-acting injectable antipsychotics, and medication for addiction treatment where indicated.
  • Follow admitted patients to the floor and document care in the hospital's electronic health record.
  • Provide continuity from hospital psychiatric consultation to outpatient bridge appointments.
  • Provide occasional weekend on-call coverage.

firsthand, a New York-based serious mental illness care provider, has secured $10 million of a planned $32 million funding round, according to public filings. The raise follows significant setbacks for the company, which was forced to cut most of its staff in March 2025 after losing a major Medicaid contract through UnitedHealthcare. Founded in 2021, firsthand uses a peer support model where individuals with lived experience of serious mental illness engage with patients at home. The company previously raised $28.1 million in its Series B from GV in February 2023. The new funding signals renewed growth momentum. firsthand has established a partnership with Carelon focusing on total cost of care for serious mental illness patients. To date, the company has raised approximately $52.9 million across three funding rounds.
