Requirements
  • Active New York State Licensed Practical Nurse license.
  • Current Cardiopulmonary Resuscitation and Basic Life Support certification.
  • Minimum of 1–2 years of nursing experience is preferred, particularly in home care, pediatrics, or geriatrics.
  • Strong clinical judgment and the ability to work independently.
  • Experience with electronic health records is preferred.
  • Reliable transportation and/or a valid driver's license is preferred.
  • Excellent communication, organizational, and interpersonal skills.
Responsibilities
  • Provide skilled nursing care, including medication administration, wound care, catheter care, and intravenous therapy.
  • Conduct patient assessments and contribute to individualized care planning.
  • Monitor patient conditions and report changes appropriately.
  • Educate patients and family members regarding treatment plans and health management.
  • Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards.
  • Ensure patient safety while upholding the highest standards of nursing practice.
  • Collaborate with interdisciplinary healthcare teams and community providers.
Desired Qualifications
  • One to two years of nursing experience in home care, pediatrics, or geriatrics.
