Requirements
  • A minimum of a Master's Degree in Nursing with a focus on Family Nurse Practitioner, Acute Care Nurse Practitioner, Adult Nurse Practitioner, Geriatric Nurse Practitioner, or Adult-Geriatric Nurse Practitioner from an accredited Nurse Practitioner program.
  • An active, unencumbered Nurse Practitioner license in the state of the job posting and residence in that state.
  • Current board certification in the field through either the American Nurses Credentialing Center or the American Association of Nurse Practitioners.
  • Basic Life Support certification and an active National Provider Identifier number as a Nurse Practitioner.
  • An active, unencumbered Drug Enforcement Administration license or eligibility to obtain one.
  • A valid driver's license with an acceptable driving record.
  • Proof of an active auto insurance policy.
  • Ability to conduct in-home visits and prior experience using a telehealth platform.
  • Proficiency with Microsoft Office, including Excel, Word, and G Suite.
  • Proficiency with Microsoft 365, Electronic Health Records, scheduling software, and RingCentral applications.
  • Ability to work independently and make sound decisions while adhering to established protocols and guidelines.
  • Organizational and time-management skills for administrative tasks and telehealth consultations.
  • Experience working with culturally diverse and underserved populations.
  • Experience diagnosing and treating ongoing, chronic, and urgent medical or psychological conditions in older adults.
  • Experience managing primary care, geriatrics, internal medicine, and palliative care.
  • Experience with palliative care and end-of-life communication.
  • Ability to communicate effectively with patients, families, and remote healthcare professionals.
  • Ability to lift up to 20 pounds; moving, lifting, or transferring patients may involve lifting up to 50 pounds and assisting with weights exceeding 100 pounds.
  • Ability to stand for extended periods and drive to patient, hospital, skilled nursing facility, or community resource locations.
  • Fine motor skills and visual acuity.
Responsibilities
  • Conduct in-home, patient-facing urgent care visits.
  • Facilitate in-home and assisted telehealth visits between clinical teams and patients, including urgent assessments and follow-up visits.
  • Complete and document thorough home safety inspections during patient visits.
  • Check and record patient vitals, including weight, blood pressure, heart rate, lung sounds, and blood sugar levels, along with health challenges identified during visits or reported by patients.
  • Provide support for patient care needs with the clinical team.
  • Update patient records and verify their accuracy at each patient visit.
  • Provide patient education with support from the clinical team.
  • Deliver quality care while meeting company metrics related to appropriate patient needs.
  • Review patient care interventions and assessments for transitions of care, end-of-life discussions, HEDIS gaps such as screening and medication adherence, and palliative care interventions.
  • Complete and submit required documentation and paperwork daily.
  • Attend Interdisciplinary Team meetings and provide information about patient or family interactions.
  • Collaborate with care team members on patient care strategies.
  • Participate in regularly scheduled interdisciplinary team meetings.
  • Attend required medication, equipment, and device training and complete ongoing training.
  • Build relationships with local community providers and caregivers.
  • Use available resources to address patient needs and make appropriate internal or external referrals.
  • Maintain required active state licensure.
  • Follow professional conduct, patient-care, and documentation standards.
  • Provide local travel to patient and community locations, including possible weekend coverage.

  • Three to five years of Nurse Practitioner experience.
  • Experience in value-based care, home care, primary care, hospice, palliative care, or geriatrics.
  • Other complex patient healthcare experience.
