Requirements
  • Must be bilingual in English and Spanish.
  • A bachelor's degree is required.
  • Experience with RMA is required.
  • Knowledge of community resources and services to support service delivery and link members with services based on a client-centered service plan.
  • Verbal and written communication skills to provide linguistically appropriate services to the assigned caseload.
  • Ability to communicate with other professionals, provider networks, and managed care organizations regarding client status, functioning, and additional service needs.
Responsibilities
  • Manage a caseload of 85 or more Health Home Care clients.
  • Assist in developing comprehensive care plans.
  • Address client service needs, including housing, benefits, medical care, transportation, education, employment, crisis intervention, and other supportive services.
  • Work as part of a care team that includes a supervisor, clinicians, verbal psychotherapists, and psychiatrists.
  • Conduct advocacy and assessments; plan services; create linkages and referrals; and maintain ongoing documentation and monitoring in electronic health records.
  • Contact clients with mental illness, substance use disorders, and chronic medical conditions that significantly affect functioning on a monthly basis, in person and by phone.
  • Work on site Monday, Tuesday, and Thursday as part of the hybrid Monday–Friday schedule.
Desired Qualifications
  • Experience with HARP clients is preferred.
