Requirements
  • National certification in Medical Assisting from AAMA, NHA, NCCT, NAHP, or a similar organization is required.
  • At least 1 year of experience working in a clinical setting is required.
  • Strong communication skills and the ability to work collaboratively with clinical staff and patients are required.
  • Proficiency in medical terminology and clinical processes is required.
  • Must be highly organized, detail-oriented, and capable of handling multiple tasks in a fast-paced environment.
  • Must be willing to travel to different clinic locations in the Greenville, Tennessee area as needed.
  • Must be available for a full-time Monday–Friday, 9:00 a.m.–5:00 p.m. Eastern Time schedule.
Responsibilities
  • Engage with eligible patients in person at partner clinics and introduce Chronic Care Management and Remote Patient Monitoring programs.
  • Educate patients on the benefits of Chronic Care Management and Remote Patient Monitoring services and answer questions about the programs.
  • Guide eligible patients through the enrollment process and ensure that all required information and documentation are completed accurately.
  • Communicate clearly and compassionately with patients and their families throughout enrollment.
  • Coordinate with clinic staff and the care management team to support a smooth patient enrollment experience.
  • Maintain accurate records of patient interactions and enrollment status.
  • Work toward established patient enrollment goals.
  • Build positive relationships with patients and clinic staff while representing the organization professionally.
Desired Qualifications
  • Experience in a care management or chronic disease management role.
