Requirements
  • Possess, at minimum, a Nursing Degree from an accredited college or university, or graduate from an approved Licensed Practical Nurse/ Licensed Vocational Nurse program.
  • Possess a current, unencumbered, active license to practice as a Licensed Practical Nurse/ Licensed Vocational Nurse in the state.
  • Read, write, speak, and understand the English language.
  • Read technical procedures and comprehend policy and procedure manuals.
  • Effectively present information and respond to questions from managers and employees.
  • Relate information concerning a resident’s condition.
  • Apply fractions, percentages, ratios, and proportions to practical situations.

  • Assist with evacuating residents during emergency situations when necessary.
Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and facility guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Prepare and submit required written and oral shift reports and recommendations.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and authorized facility surveys.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Admit, transfer, and discharge residents as required.
  • Complete medical forms, reports, evaluations, studies, charting, accident and incident reports, census reports, transfer forms, and other required documentation.
  • Maintain resident records and chart nursing notes, physician orders, diet changes, treatments, and care provided.
  • Receive telephone orders from physicians and transcribe orders to resident charts, medication cards, treatment plans, and care plans.
  • Prepare and administer medications as ordered by physicians and verify resident identity before administration.
  • Maintain adequate medication, supply, and equipment stock and ensure narcotic records are accurate.
  • Report medication, narcotic, charting, order, accident, incident, safety, and suspected fraud discrepancies or violations.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend staffing actions to the Nurse Supervisor.
  • Develop work assignments, provide leadership to assigned nursing personnel, conduct unit rounds, and report problem areas.
  • Ensure nursing assistants and nurse aide trainees meet applicable training and supervision requirements.
  • Receive and give nursing reports at the beginning and end of shifts.
  • Inform nursing personnel about new admissions and prepare rooms for admissions.
  • Orient new residents and family members to the facility.
  • Coordinate physician rounds, diagnostic and therapeutic services, resident care, treatment, rehabilitation, and social or activity programs.
  • Review resident charts and care plans, monitor resident condition, and ensure prescribed treatments are administered properly.
  • Provide or oversee licensed nursing care and professional services, including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, specimen collection, vital signs, and range-of-motion exercises.
  • Notify physicians and next of kin about accidents, incidents, and changes in resident condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Maintain clean, safe, and sanitary nursing areas and ensure compliance with infection-control, hand-hygiene, fire-safety, disaster-preparedness, and personal-protective-equipment procedures.
  • Participate in staff orientation, in-service training, clinical supervision, continuing education, and professional development programs.
  • Recommend unit equipment and supply needs and ensure safe, authorized, and efficient equipment use.
  • Review resident care plans daily and coordinate revisions with the Nurse Supervisor and Resident Assessment/Care Plan Coordinator.
  • Assist with MDS planning, scheduling, revisions, and implementation of RAPs and Triggers.
  • Provide requested data to the Quality Assurance and Assessment Committee.
  • Maintain confidentiality of resident protected health information and facility information-system credentials.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
