Responsibilities
  • Provide assigned residents with routine daily nursing care and services in accordance with each resident’s assessment and care plan and supervisors’ directions.
  • Identify residents using wristbands or photo card files before treatments, meal service, and other care as necessary.
  • Use only facility-authorized abbreviations when recording information.
  • Promptly report changes in residents’ conditions to the Nurse Supervisor or Charge Nurse.
  • Make informative, descriptive entries on flow sheets, notes, charts, and other records; record and report observations and data as instructed.
  • Report accidents and incidents observed during the shift in which they occur.
  • Protect residents’ health information, promptly report suspected or known unauthorized disclosures, and report suspected unauthorized attempts to access the facility’s information system.
  • Perform only nursing care procedures and use only equipment for which training has been provided; report defective equipment to the Nurse Supervisor or Charge Nurse.
  • Prepare residents’ rooms for admission, greet and escort residents to their rooms, introduce them to roommates and personnel as appropriate, and make them comfortable.
  • Inventory and mark residents’ possessions, store clothing, help pack possessions for transfers or discharge, transport residents to new rooms or receiving areas, and assist with vehicle loading and unloading as necessary.
  • Maintain a warm, calm environment with personal attention and positive emphasis throughout the unit and shift.
  • Meet regularly with nursing personnel on the shift to help identify and correct problem areas and improve services.
  • Report residents’ complaints and grievances and participate in and receive nursing reports as instructed.
  • Follow established policies for exposure to blood and body fluids; report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals under facility procedures.
  • Make beds and add covers as requested.
  • Measure and record residents’ temperatures, pulses, and respirations; weigh and measure residents as instructed.
  • Frequently check residents who cannot call for help, promptly answer calls, and routinely check that personal care needs are met in accordance with residents’ wishes.
  • Assist residents with food arrangements, record food and fluid intake, report changes in eating habits, keep water pitchers clean and filled with fresh water within reach, and provide after-meal care.
  • Complete assigned tasks according to facility policies and procedures, supervisors’ instructions, work assignments, and schedules; cooperate with other departments and facility personnel to maintain nursing services.
  • Notify the facility when late or absent.
  • Assist residents with dental and mouth care, bathing, hair care, dressing and undressing, and changing wet or soiled gowns, clothing, and linens.
  • Help residents prepare for medical tests, including laboratory work, X-rays, therapy, and dental procedures.
  • Lift, turn, move, position, and transport residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts.
  • Perform restorative and rehabilitative procedures as instructed and assist in preparing residents for physical examinations.
  • Observe and report pressure areas, skin breakdown, and injuries of unknown source, including skin tears.
  • Observe disoriented and comatose residents and record and report relevant data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse Supervisor or Charge Nurse if a resident leaves or is missing from the facility.
  • Report all allegations of resident abuse and misappropriation of resident property.
  • Honor residents’ refusals of treatment and report such requests to a supervisor.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. He previously held CFO roles at Capital Senior Living Corporation, managing 128 communities across 23 states, and Belo Corp. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Under his leadership, the company expanded to 323 facilities across 17 states, serving over 31,700 patients daily and generating $5.29 billion in revenue in 2025, representing 29.3% year-on-year growth. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
