Requirements
  • Two years of management or supervisory experience within the past five years in a health care setting, preferably in home health.
  • Graduation from an accredited school of nursing.
  • Current California Registered Nurse license.
  • Current Basic Life Support card.
  • Valid California driver's license with an acceptable driving record; licenses and certifications must be current at hire and maintained throughout employment.
Responsibilities
  • Provide field oversight of clinicians who provide care and treatment to patients in their homes.
  • Handle incoming calls promptly and follow up to ensure appropriate actions meet internal and external customer needs.
  • Coordinate training and educational programs for field nurses to improve clinical outcomes and competencies.
  • Assess staff learning needs and formulate plans and goals for the next evaluation period with employees; perform administrative call duties and serve as a role model and resource for staff.
  • Serve as the first point of contact for field clinicians needing assistance with patient-care escalations.
  • Coordinate at least biweekly case conferences with the clinical team to discuss patient progress and revise care plans as needed to meet planned outcomes.
  • Review and evaluate each case by reviewing clinician services, conferences, and records; discuss and verify impressions; instruct and guide clinicians to promote effective performance and quality home care; and remain available during operating hours to assist clinicians.
  • Review patients' clinical diagnoses, medications, procedures, and clinical courses.
  • Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plans of care.
  • Coordinate and manage patient care through a team of Registered Nurse case managers and Licensed Vocational Nurses.
  • Participate in identifying, developing, designing, and implementing nursing education programs; determine success measures and evaluate program effectiveness to support staff development and patient and customer satisfaction.
  • Meet regularly with customers to maintain relationships and provide updates, including readmission reports, and offer insights to improve patient outcomes and reduce readmissions.
Desired Qualifications
  • A bachelor's degree is preferred.
  • Home health experience is preferred.
