Requirements
  • Registered Dietitian registration through the Commission on Dietetic Registration.
  • Current state licensure if applicable.
  • At least one year of professional work experience in clinical nutrition as a Registered Dietitian.
  • Demonstrated ability to teach, organize work, communicate effectively, provide customer service, develop relationships, achieve results, build teams, and make decisions.
  • Ability to perform responsibilities with minimum supervision.
  • Basic computer skills.
  • Maintain dietetic registration and continuing education hours as specified by the American Dietetic Association and applicable state licensure regulations.
Responsibilities
  • Provide nutritional services for in-center and home patients as applicable to maximize nutritional status and improve clinical outcomes.
  • Assess patients’ knowledge of diet and kidney disease and provide education appropriate to their learning needs.
  • Provide patients, families, and caregivers with detailed education regarding nutritional status, adequacy measures and results, fluid status, and nutrition-related laboratory parameters.
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments.
  • Participate with the interdisciplinary team in developing written, individualized comprehensive plans of care.
  • Calculate diet prescriptions according to standards of practice.
  • Complete progress notes for chronic dialysis patients and maintain accurate medical-record documentation in accordance with company policies and applicable regulations.
  • Provide ongoing nutrition assessment and outcome-oriented nutrition counseling.
  • Identify malnourished patients and those at risk for malnutrition, and collaborate with the interdisciplinary team on interventions, resources, and solutions.
  • Review each patient’s albumin level and body weight at least monthly and implement the plan of care with patient, family, or caregiver involvement.
  • Monitor adherence and response to nutrition therapy, adjust therapy as needed, document barriers to goal achievement, and implement corrective plans of care.
  • Collaborate with the interdisciplinary team on urea kinetic modeling recommendations to achieve the physician’s Kt/V prescription.
  • Collaborate on management and recommendations involving mineral metabolism, renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status, and sodium/volume control.
  • Adhere to company algorithms and medical-record policies.
  • Report nutrition quality-assessment and improvement results and participate in the interdisciplinary quality-assessment and improvement program.
  • Coordinate with team members to provide pre-end-stage renal disease education as appropriate.
  • Work with the Case Manager to coordinate patient-education plans and identify candidates for referral to a physician for nutritional supplements when appropriate.
  • Review nutrition-related laboratory results, counsel patients or caregivers, formulate actions, recommend treatment changes, and communicate results and recommendations to physicians, facility staff, and other care providers.
  • Participate in staff, care-plan, quality-improvement, and other applicable meetings.
  • Consult with the corporate dietitian regarding nutrition concerns as needed.
  • Maintain and improve knowledge and skills for competent practice.
  • Collaborate with the interdisciplinary team to ensure work areas are safe and clean.
  • Travel between assigned facilities and various community locations, and attend regional, business-unit, or corporate meetings when required.
  • Perform other nutrition-management and related duties as assigned.
Desired Qualifications
  • Previous renal experience.
  • Board Certified Specialist in Renal Nutrition certification.
