About the Company

Optimal Care

Medicare-certified home health with geriatric care

Optimal Care provides Medicare-certified home health services for seniors using a nurse practitioner-led care model. A nurse practitioner visits patients at home to assess needs and develop a personalized plan that includes programs such as behavioral health, dementia, geriatric, diabetic, wound, and orthopedic care. This setup delivers a wide range of specialty care directly in the home, not just post-surgical or rehabilitation services. Its goal is to keep older adults safe and healthy at home through coordinated, in-home care.
