  • A bachelor's degree or higher from an accredited school of Nursing is required.
  • Five years of hospital clinical nursing experience, including three years in case management, is required.
  • An active Texas Registered Nurse license is required; a permanent Texas license must be obtained within 60 days if establishing Texas residency.
  • A Magnet ANCC-recognized case management certification is required: ACHPN-HPCC, CCM, CMC, ACM-NBCM, CDCES, CHPN-HPCC, CMGT-BC, CM-ABOHN, CMCN, ANCC-NCM, CMC-NACCM, COHN, COHN/CM, or CPHRM.
  • Demonstrate the skills and competencies necessary to safely perform the assigned job, as determined through ongoing skills, competency assessments, and performance evaluations.
  • Have sufficient proficiency in speaking, reading, and writing English to perform the essential job functions, especially those affecting patient or employee safety or security.
  • Demonstrate comprehensive knowledge of Medicare, Medicaid, and managed care requirements.
  • Demonstrate comprehensive knowledge of community resources, health care financial and payer requirements and issues, and eligibility for state, local, and federal programs.
  • Demonstrate comprehensive knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
  • Understand pre-acute and post-acute care settings and post-acute community resources.
  • Work independently and demonstrate assessment, organizational, and problem-solving skills to prioritize multiple tasks and role components.
  • Use critical thinking and evidence-based analysis when interacting with physicians, payers, patients, and families.
  • Demonstrate competency with Microsoft Office, including Access, Excel, Outlook, PowerPoint, and Word.
Responsibilities
  • Plan case management comprehensively for a targeted patient population on designated units or service lines, including care transitions and discharge planning.
  • Identify areas needing performance improvement, including daily workflows, education, processes, and patient satisfaction.
  • Manage discharge planning and continuity of care in collaboration with patients, families, physicians, and interprofessional healthcare teams to facilitate quality care and desired treatment outcomes.
  • Serve as a preceptor for case managers and social workers.
  • Collaborate with physicians and the multidisciplinary healthcare team to monitor patient medical progress and ensure the plan of care and services are patient-focused, high-quality, efficient, and cost-effective.
  • Communicate with physicians, care teams, patients, families, and payers to ensure safe and timely care transitions; seek physician-advisor assistance as needed and facilitate payer conversations to expedite authorization for appropriate post-acute placement.
  • Implement staff education for patient populations and department processes; coach and mentor staff and nursing students; serve as a case management resource; and educate physicians, nurses, and other healthcare providers on case management topics.
  • Contribute to employee engagement and peer-to-peer accountability.
  • Identify discharge barriers, anticipate and manage emergent situations, and facilitate timely assessment and intervention to reduce readmissions, complete treatment and discharge plans, modify plans of care as clinical conditions change, assign appropriate levels of care, complete required documentation, and eliminate barriers to discharge.
  • Advise and support care team members in managing complex psychosocial or medical conditions; formulate discharge plans to reduce avoidable delays and escalate barriers to leadership or the physician advisor.
  • Maintain knowledge of and ensure compliance with CMS Conditions of Participation and other rules and regulations affecting case management; facilitate referrals and transfers for home health care, hospice, durable medical equipment, and post-acute services; consider social determinants of health in decisions.
  • Review charts for appropriate level of care, medical necessity for continued hospitalization, and discharge readiness; document assessments and interventions; and take action to improve care and facilitate discharge.
  • Conduct post-discharge inpatient-record reviews for quality-indicator compliance; support real-time compliance with CMS guidelines and other performance measures; report compliance, problem areas, and corrective actions to appropriate parties; and partner with Business Practice.
  • Conduct peer-to-peer evaluations, track and trend noncompliance, formulate appropriate PDCA, and report quality-of-care concerns to the appropriate governing body.
  • Identify opportunities to improve patient satisfaction in discharge and care coordination and collaborate with department leadership on evidence-based patient engagement strategies.
  • Manage length of stay for assigned cases, identify and address trends affecting length of stay or treatment progress, and contribute to hospital and department financial targets.
  • Support timely discharges for uninsured patients and hospitalized outpatients and inform physicians of disposition options that maximize patient benefits and hospital reimbursement.
  • Partner with case management leadership on length-of-stay management and medical appropriateness of resource utilization.
  • Identify opportunities for practice changes and, with management, implement evidence-based practice and performance-improvement projects and shared-governance activities.
  • Identify self-development needs, take appropriate action, complete career discussions with management, and maintain the My Development Plan.
