Requirements
  • A Bachelor's degree in Psychology, Sociology, Human Services, or a related field is required.
  • At least one year of experience in social services or behavioral health is required.
  • The employee must complete required documentation and assigned tasks accurately and on time.
  • The employee must demonstrate cultural competence and understanding of age-specific needs and human development across the lifespan.
  • The employee must subscribe to recovery and trauma-informed care models.
  • The employee must complete the organization's mandatory COVID-19 and flu vaccination requirements.
Responsibilities
  • Attend staff meetings and trainings, complete documentation and assigned tasks with timeliness and accuracy, and maintain positive relationships with internal teams and external partners.
  • Deliver client-centered behavioral health services, including case management, life-skills support, and social services.
  • Develop and implement individualized treatment goals and plans.
  • Coordinate services, referrals, discharge planning, and support for successful community reintegration to ensure continuity of care.
  • Maintain accurate and compliant daily logs, incident reports, progress reports, court-related documentation, and required regulatory forms in accordance with organizational and legal standards.
  • Facilitate educational groups, monitor behavioral and health changes, assist with activities of daily living such as meals and housekeeping, and collect basic health data such as vital signs and specimens as needed.
  • Perform admissions procedures, continuity-of-care activities, and client reengagement.
  • Serve as a liaison between the facility, clients, and their families, and coordinate services within the crisis-care continuum and post-crisis stabilization supports.
