Requirements
  • Bachelor’s degree in related field of study required.
  • Competency in co-occurring disorders, including ability to engage, screen, deescalate and coordinate care.
  • Two years mental health experience, to be able to independently assess person’s served needs and to implement and evaluate effective plans of care and acquire and effectively utilize knowledge of policies and procedures.
Responsibilities
  • Engage with individuals to assess and prioritizes person’s served treatment and service needs.
  • Develops and implements plan of care and collaborates with the clinical team to assure a uniform plan of care.
  • Provides input to the interdisciplinary treatment plan, functioning as a team member.
  • Establishes therapeutic relationship with assigned person’s served including engaging, deescalating, screening, and managing crises.
  • Serves as a liaison between community resources, family, person’s served, and hospital- or clinic-based providers of care.
  • Responsible to document and report all necessary information according to established policies and procedures.
