Responsibilities
  • Provide optimal pharmaceutical care and develop, implement, and maintain systems that promote desirable patient outcomes through safe and cost-effective drug therapies.
  • Monitor drug therapies, including drug-herb, drug-drug, and drug-food interactions; contraindications; allergies; and appropriateness of medications and doses.
  • Interpret verbal and written medication orders, transcribe orders to computerized patient medication profiles, and maintain and reconcile accurate and complete profiles.
  • Sustain the formulary by minimizing non-formulary procurements, using therapeutic substitution protocols, and promoting rational drug therapy selection.
  • Review and interpret laboratory data related to therapeutic drug monitoring; detect, report, and document suspected adverse drug reactions; and assess renal drug dosing based on patients’ renal function.
  • Document clinical activities and interventions accurately and completely.
  • Provide timely drug information and clinical consultation to professional staff and practitioners, and recommend appropriate, cost-effective therapeutic alternatives to medical staff.
  • Coordinate and supervise direct support personnel.
  • Apply evidence-based therapeutic guidelines to prevent and treat disease; assess medication regimens to identify, resolve, and prevent medication-related problems and promote optimal outcomes.
  • Assess medication dosing, age-specific considerations, pharmacokinetics, antibiotic management, therapeutic drug monitoring, and core-measure compliance, and make recommendations to providers.
  • Attend clinical rounds when applicable and support graduate medical education by attending cases and didactic activities with residency staff as assigned.
  • Research and review medical literature and maintain current knowledge of developments in drug therapy.
  • Interact with patients, families, and healthcare providers to support continuity of services and safe, timely, cost-effective medication therapy.
  • Oversee the preparation and storage of sterile inpatient and outpatient preparations in the Outpatient Infusion Unit, ensuring compliance with USP 797, professional practice standards, and applicable laws and regulations.
  • Manage the preparation, storage, and handling of hazardous materials, including spills.
  • Review parenteral nutrition orders daily for appropriateness and safety, monitor relevant laboratory values, osmolarity, and other parameters, and recommend therapy optimization to prescribers in conjunction with nutrition services.
  • Optimize chemotherapy regimens by reviewing indication, dose, route, preparation, and adjuvant medications, including hydration; monitor drug interactions and laboratory values for toxicity; and oversee safe, accurate chemotherapy preparation.
  • Oversee dispensing sterile products from IV services and directly supervise support staff.
  • Work with the Inventory Manager to ensure appropriate acquisition and stock points for high-cost items.
  • Generate electronic charges to facilitate billing and financial services.
  • Oversee cleaning and maintenance of the clean room, ante-room, and laminar flow hoods.
  • Maintain current pharmacotherapeutic knowledge focused on oncology, nutrition, and biologicals, and maintain references and related policies and procedures for sterile preparations and chemotherapy.
  • Lead quality improvement processes in IV Services.
  • Perform other related duties as assigned.

Care New England cuts 30+ leadership jobs to close $20M budget gap

Care New England is cutting more than 30 leadership and non-clinical positions to address a projected $20 million budget deficit for fiscal year 2026. The announcement comes as hospitals nationwide face financial pressures from federal Medicaid funding changes and policy overhauls. CEO Dr Michael Wagner cited uncompetitive state reimbursement rates compared with neighbouring states as factors hindering staff recruitment and retention. The health system, Rhode Island's second-largest hospital operator, employs over 8,000 people across Butler, Kent and Women & Infants hospitals. Care New England has faced ongoing financial challenges, exacerbated during the pandemic. A proposed 2022 merger with competitor Lifespan was blocked by regulators on anti-trust grounds. The company spent more than $10.8 million on executive compensation in fiscal 2024.
