Requirements
  • Hold an Illinois Registered Nurse license.
  • Have a minimum of 3 years of home health clinical experience.
  • Hold an Illinois driver’s license and maintain car insurance.
Responsibilities
  • Develop and implement quality and performance improvement activities.
  • Conduct medical-record reviews, including plans of care, clinical notes, and progress notes, for quality and compliance standards.
  • Facilitate interdisciplinary case conferences while monitoring coordination of care and OASIS data.
  • Complete OASIS data submissions and monitor OASIS data integrity.
  • Ensure the development, implementation, and updating of individualized plans of care.
  • Monitor and ensure the accuracy of publicly reportable data.
  • Produce monthly reports monitoring the quality of services delivered.
  • Perform direct patient care to maintain clinical competence and serve as a clinical resource to staff.
  • Assist with staff onboarding, training, and educational needs assessments.
  • Assist with evaluating clinical competence and compliance.
  • Support department customer-service and financial goals.
  • Conduct assessments and maintain clinical policies and procedures.
  • Support the director with operational department functions as needed.
  • Perform related electronic medical-record tasks and functions.
  • Promote a clean and safe environment of care using SAFE error-prevention habits.
  • Maintain patient and coworker privacy and confidentiality by abiding by the hospital’s Standards of Conduct.
Desired Qualifications
  • Hold COS-C or COQS certification.
