Requirements
  • Graduate from an accredited Physician Associate or Nurse Practitioner program.
  • Hold licensure as an advanced practice clinician (Physician Assistant or Nurse Practitioner) in Delaware, or be able to obtain it with an offer letter.
  • Hold Drug Enforcement Administration licensure or be able to obtain it before credentialing.
  • Maintain current Basic Life Support certification according to American Heart Association guidelines.
Responsibilities
  • Perform comprehensive assessments for new and established oncology and hematology patients.
  • Manage complex oncology and hematology patients, including side-effect management, diagnosis, and treatment.
  • Apply critical thinking, prioritization, and time-management skills based on patient assessments and interpretation of test results.
  • Serve as primary clinical support for the infusion center when assigned, including managing infusion reactions, addressing patient-care needs raised by nursing and pharmacy, covering triage issues and assessments, and responding to critical laboratory and radiology results.
  • Formulate and implement survivorship care plans for curable cancer patients.
  • Identify and initiate referrals to palliative care.
  • Collaborate with the palliative care team to manage patients' ongoing care.
  • Communicate with patients, families, physicians, and the multidisciplinary oncology team.
  • Coordinate patient care with primary care and specialty providers when appropriate.
  • Provide patients and families with detailed education and counseling regarding diagnosis, therapy goals, expectations, and treatment-related side effects and management.
  • Develop comprehensive plans of care with the attending oncologist, including selection of appropriate pharmacologic agents, referrals, and end-of-life care.
  • Provide inpatient oncology and hematology acute-care coverage, including rapid assessment and treatment of acutely ill oncology and hematology patients.
  • Collaborate with the medical oncologist and inpatient disciplines to determine the most appropriate level of care.
  • Assist with admission and discharge of patients receiving inpatient chemotherapy in collaboration with the attending oncology physician.
  • Administer intrathecal chemotherapy in collaboration with interventional radiology.
  • Provide after-hours call coverage on a rotating schedule, including telephone triage, clinical decision-making, and urgent inpatient and outpatient concerns in conjunction with the on-call oncologist.
  • Perform other duties as assigned.
