  • Have at least 5 years of experience practicing supportive oncology care, including direct patient care, and be comfortable being accountable for the clinical care of the teams.

  • Be available 4 or more days per week; a minimum of 3 days per week is required.

  • Lead the care team and deliver supportive care to patients with cancer, including supportive oncology patient care, palliative care, second opinions, clinical trial advisory, and documentation integrity.
  • Care for high-acuity and clinically complex patients across the hybrid model, including home-based and virtual visits, and manage clinical escalations from the care team.

  • Uphold clinical policies, documentation standards, and quality measures in daily practice, and help the team consistently deliver on them.
  • Partner with the Market Medical Director and national clinical leadership to refine care pathways, workflows, and programs, bringing frontline insight into the care model for complex, seriously ill, and home-based patients.
Desired Qualifications

  • Be a natural mentor and clinical leader who elevates the practice of nurse practitioners, registered nurses, and other care team members.
  • Be comfortable with technology.

Daymark Health partners with health plans to take on total cost of care risk and provides supplementary cancer care through in-home and virtual services. Its platform blends care navigation, 24/7 support, symptom management, and early palliative care via a twice-weekly symptom tool, delivered by a community team of nurse practitioners, nurses, social workers, and Health Partners. The model is tied to payer contracts and aims to keep patients outside traditional clinics, reducing fragmentation and coordinating care across the health system. The goal is to improve patient outcomes while lowering overall costs for payers and patients, addressing the needs of the roughly two million Americans diagnosed with cancer each year.
