Requirements
  • Graduate from an accredited school of professional nursing.
  • Possess a current active license to practice nursing in the Commonwealth of Pennsylvania.
  • Have two years of experience as a professional nurse in nursing practice, or previous community health and/or hospice care experience as a professional nurse.
  • Demonstrate comprehensive knowledge of professional nursing theory and ability to apply it in a community health setting.
  • Communicate effectively orally and in writing with patients, families, physicians, and related persons.
  • Instruct nonprofessionals in the care and treatment of physically and emotionally ill patients.
  • Prepare neat, timely, accurate, and concise records and reports.
  • Use community resources and apply hospice care principles.
  • Understand social service and counseling principles related to death and dying.
  • Meet the physical, sensory, environmental, and safety requirements of heavy work, including lifting, standing, walking, stooping, handling, and travel to patient homes and clinics.
  • Maintain good physical and mental health and comply with uniform, confidentiality, safety, attendance, and dress-code policies.
Responsibilities
  • Determine eligibility for admission, obtain financial and statistical data, and formulate an initial care plan according to the patient's condition.
  • Collaborate with the interdisciplinary team to ensure appropriate care planning.
  • Communicate orally and in writing with the patient's physician and/or hospice Medical Director as required.
  • Provide counsel, advice, and training to hospice volunteers when working with patients and families.
  • Keep informed of current changes and trends affecting hospice care and professional nursing functions.
  • Participate in the bereavement program and supervise volunteers providing bereavement care.
  • Provide comprehensive nursing care, health teaching, guidance, and supervision to individuals and families while considering psychosocial, environmental, spiritual, and financial needs.
  • Supervise patient care provided by licensed practical nurses and/or home health aides.
  • Maintain patient clinical records, reports, and written communications according to agency policy, with timely and accurate documentation of goals, plans, and contacts.
  • Collaborate with the interdisciplinary team to develop, implement, and evaluate bereavement plans of care.
  • Keep administration informed about changing community needs and unavailable community resources.
  • Participate in the orientation and guidance of new staff.
  • Interpret agency services to families and community groups.
  • Contribute to the evaluation of service programs and the continuing development of staff members.
  • Participate in the development of in-service programs and group teaching for staff and other groups as assigned.
  • Maintain confidentiality and properly release information.
  • Comply with Network and departmental employee, patient, environmental safety, and reporting policies.
  • Demonstrate the Network's Service Excellence Standards in interactions with internal and external customers.
  • Demonstrate performance improvement in clinical care, customer/service improvement, operational systems/processes, and safety.
  • Use resources effectively and efficiently in daily procedures, processes, and practices.
  • Demonstrate competency in patient assessment, treatment range, growth and development, and age-appropriate communication.
Desired Qualifications
  • A Bachelor of Science in Nursing is preferred.
