Requirements
  • A High School diploma or General Educational Development credential is required.
  • The candidate must meet Center for Medicare and Medicaid Services-approved state and/or national certification requirements within the required state or CMS timeline.
  • The candidate must complete all applicable state licensure, education, and training requirements.
  • The candidate must demonstrate commitment to the organization’s culture, values, and customer service standards.
  • Previous patient care experience in a hospital setting or related facility is preferred.
  • Continued employment requires successful completion of the organization’s dialysis training program and cardiopulmonary resuscitation certification.
  • The employee must be able to perform direct patient care involving heavy lifting, moving patients, assisting with ambulation, prolonged standing, bending, and lifting or lowering solutions and equipment within the stated weight limits.
  • The employee must complete the Ishihara Color Blindness test as a condition of employment; failure does not preclude employment and reasonable accommodations may be considered.
Responsibilities
  • Provide safe and effective dialysis therapy as part of the dialysis health care team under the direct supervision of a licensed nurse.
  • Set up and operate hemodialysis machines and maintain a safe and clean working environment.
  • Provide patient-specific education regarding adequacy measures, disease processes, and vascular access under appropriate supervision.
  • Welcome assigned patients and inquire about their wellbeing since their last treatment.
  • Evaluate vascular access, perform vascular access cannulation, and administer heparin when delegated or allowed by state law.
  • Obtain pre-treatment and post-treatment vital signs and weights and perform vascular access evaluations.
  • Initiate dialysis treatment according to prescribed orders, including blood flow and dialysate flow.
  • Evaluate intradialytic problems and provide interventions according to physician orders or direction.
  • Monitor patients’ responses to dialysis therapy.
  • Perform reinfusion and discontinue dialysis treatments according to established procedures, including documentation and evaluation of venous access.
  • Obtain hemostasis and apply appropriate dressings.
  • Evaluate patients before discharge.
  • Calculate and enter individual dialysis machine programming for ultrafiltration goals, treatment time, and ultrafiltration modeling as prescribed.
  • Ensure appropriate safety alarms, including Narrow Venous Limits 160, are enabled.
  • Record accurate and timely vital signs and treatment parameters on the Hemodialysis Treatment Sheet.
  • Document patient treatment information, including completion of the Hemodialysis Treatment Sheet and progress notes when applicable.
  • Report significant information or changes in patient condition to the registered nurse or supervisor.
  • Observe patients and perform machine safety checks according to facility policy, reporting unusual findings to the registered nurse or supervisor.
  • Perform and document interventions for unusual patient status and document patients’ responses.
  • Evaluate vascular access during treatment, including arterial and venous monitoring pressures, and provide, document, and report interventions or unusual findings.
  • Obtain prescribed laboratory testing and prepare specimens for collection.
  • Clean blood spills immediately with appropriate disinfectant according to facility policy.
  • Prepare, organize, and use supplies and equipment efficiently to prevent wastage.
  • Monitor patients performing self-care under registered nurse supervision.
  • Assist in training applicable direct patient care staff on appropriate programs under nurse supervision.
  • Follow organizational business policies, procedures, systems, and ethical business practices.
  • Maintain environmental integrity and aesthetics and ensure all areas are safe and clean.
  • Clean and disinfect dialysis machines, chairs, equipment, and surrounding areas between treatments.
  • Prepare for dialysis treatments, document as appropriate, and perform required pre-treatment dialysis machine alarm testing, including Pressure Holding testing.
  • Initiate the Solution Delivery System.
  • Complete and document ongoing participation in Continuous Quality Improvement activities.
  • Enter treatment data accurately and timely into the designated clinical application.
  • Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure omitted entries are completed or corrected by appropriate staff.
  • Prepare initial patient charts for admission and compile paperwork for placement in the chart.
  • Prepare and print laboratory requisitions for prescribed or stat specimens according to laboratory destination.
  • Collect, label, prepare, and store laboratory samples according to laboratory specifications.
  • Ensure laboratory specimens are collected by the appropriate laboratory courier.
  • Assist in collecting information for infection control audits.
  • Support staff and patient adherence to infection control practices and follow infection control policies and procedures.
  • Participate in and reinforce infection control education for patients and families under the direction of the Clinical Manager or designee as allowed by state law.
  • Follow newly admitted patients through their first four weeks of dialysis and coordinate their transitional dialysis care.
  • Reinforce education and care related to new patients as allowed by state law.
  • Liaise with appropriate staff members to support the patient experience and ensure disciplines are scheduled according to plan.
  • Assist patients in understanding home dialysis products, the benefits of home dialysis, and how home dialysis can work for them.
  • Set up and perform treatments on all machines used in the Transitional Care Unit.
  • Assist patients in understanding treatment options, including demonstrating peritoneal dialysis setup.
  • Assist with coordinating the transition to the patient’s chosen dialysis modality.
  • Assist with supply ordering and inventory for the Transitional Care Unit.
