- Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families.
- Educate patients and families about hospice care, including what it includes and how it works.
- Complete documentation and coordinate the intake and admission process to meet legal, clinical, and payer requirements.
- Serve as a liaison between referral sources and the hospice care team to ensure seamless handoffs, clear communication, and rapid responses.
- Visit hospitals and facilities daily and maintain visibility with discharge planners, case managers, and physicians.
- Build and maintain relationships with referral partners and identify outreach opportunities to increase awareness of hospice services.
- Bachelor of Science in Nursing degree with five or more years of hospice experience.
- Prior experience as a discharge planner, case manager, or educator.
