Requirements
  • A Doctor of Pharmacy degree is preferred, or a Bachelor of Science in Pharmacy degree is required.
  • A current Oregon pharmacist license is required.
  • American Heart Association Basic Life Support certification is required within 90 days of hire.
  • American Heart Association Advanced Cardiovascular Life Support certification is required within 6 months of hire.
  • Proficiency in hospital computer systems, software programs, and applicable office equipment.
  • Knowledge of current federal, state, and local laws and regulations pertaining to pharmacy practice.
  • Accuracy in mathematical calculations and dispensing activities.
  • Ability to communicate clearly and effectively, verbally and in writing, with pharmacy staff, other healthcare professionals, and the public.
  • Ability to maintain confidentiality and work independently with minimal supervision.
  • Ability to organize and prioritize workload, handle multiple tasks, and make critical decisions to provide timely pharmacy services.
  • Knowledge of how to assess patient needs, incorporating age- and disease-specific characteristics to optimize pharmacotherapy.
  • Medical literature evaluation skills and understanding of evidence-based medicine.
  • Regular, consistent, and punctual attendance at the assigned job location.
  • Ability to sit extensively, with periodic standing and walking; lift up to 20 pounds; use a computer, phone, and office equipment; maintain adequate visual acuity and ability to grasp and handle objects; and communicate effectively by reading, writing, and speaking in person or by telephone.
Responsibilities
  • Perform duties within the scope of practice for a Registered Pharmacist as defined by the Oregon State Board of Pharmacy.
  • Provide clinical pharmacy patient care to optimize medication therapy and promote disease prevention, health, and wellness.
  • Perform drug utilization reviews and identify medication-therapy problems by evaluating medication orders against patient records and consulting prescribers when appropriate.
  • Assess and monitor patient drug-therapy laboratory tests according to clinical policies and procedures, communicating with prescribers or other relevant healthcare practitioners.
  • Monitor patients and review profiles to identify, prevent, or mitigate medication-therapy problems, including improper drug or dose selection, untreated indications, medications without indications, therapeutic duplications, treatment failures, drug interactions, and adverse drug events or reactions.
  • Provide clinical consultation and clarification to practitioners; recommend evidence-based medication regimens and monitoring plans; and suggest appropriate, cost-effective therapeutic alternatives to medical staff.
  • Participate in medication reconciliation by verifying medication histories obtained by pharmacy technicians, resolving medication discrepancies, communicating with patients and providers, and documenting interventions.
  • Counsel patients and their families and provide drug information.
  • Participate in pharmacy operations and medication dispensing.
  • Oversee and provide ongoing supervision for pharmacy technicians, interns, and other support personnel to ensure safe and accurate drug preparation, distribution, and control.
  • Follow policies and procedures for preparing and supervising sterile and non-sterile compounding.
  • Participate in managing medical emergencies.
  • Document clinical activities and interventions accurately and completely.
  • Participate in education and training for healthcare practitioners, including students, interns, nurses, technicians, and other healthcare professionals.
  • Participate in quality initiatives by monitoring processes, documenting, and analyzing data.
  • Collaborate with healthcare-team members to develop and implement clinical initiatives.
  • Participate in department and staff meetings and committees.
  • Perform other duties as required or assigned.
Desired Qualifications
  • A minimum of 3 years of relevant hospital experience is preferred.
  • A Doctor of Pharmacy degree is preferred.
