Requirements
  • One of the following is required: PhD or PsyD, MSW with LCSW, or LPC/LMFT, with an Oregon license required.
  • Experience in behavioral health counseling, mental health assessment, substance use intervention, crisis intervention, and case management.
  • Demonstrated understanding of therapeutic interventions in primary care, including cognitive behavioral therapy, motivational interviewing, solution-focused and person-focused interventions, adverse childhood experiences, and their effects on health outcomes and health-seeking behaviors.
  • Demonstrated understanding of trauma-informed and patient-centered care in a medical home.
  • Ability to work independently using critical thinking, problem-solving, and time-management skills.
  • Skill in developing and maintaining professional relationships and navigating differing care strategies.
  • Ability to use technology supporting in-person and telehealth services, including electronic health records, virtual meeting platforms, and pagers.
  • Knowledge of primary care behavioral health consultation principles and practices, including assessment, diagnosis, treatment planning, counseling strategies, and population-based preventive and chronic care.
  • Demonstrated ability to practice with minimal supervision using sound clinical judgment.
Responsibilities
  • Provide two days of direct patient care per week as an integrated behavioral health clinician, including interventions for anxiety, depression, behavior change needs, eating disorders, insomnia, substance use disorders, and other mental health conditions.
  • Proactively identify patients, provide same-day warm handoffs, conduct scheduled follow-ups and assessments, consult with care team members, and perform crisis intervention with safety planning.
  • Participate in behavioral health case management as part of a multidisciplinary team, coordinating short- and long-term mental health care with internal and community resources.
  • Document services appropriately in the electronic health record according to established workflows.
  • Maintain technology fluency for in-clinic and telehealth care and remain accessible through department-standard communication methods.
  • Support behavioral health team staff across the Richmond and East Portland Federally Qualified Health Center clinics.
  • Work with the Department of Family Medicine Behavioral Health Leadership Team to guide delivery of integrated behavioral health services based on community needs and clinic resources.
  • Work with assigned clinics and clinic leadership to ensure adherence to service excellence, ethical, and compliance guidelines.
  • Ensure clinics are staffed to their allotted full-time-equivalent levels, maintain accurate position descriptions, screen applicants, organize interviews, onboard new staff, and advocate for additional staffing when appropriate.
  • Provide clinical support and direct supervision to behavioral health staff across assigned primary care clinics and address personnel issues, training, onboarding, and clinical care delivery with clinic leadership.
  • Provide clinical supervision to pre-licensed behavioral health consultants pursuing licensure and identify formal and informal consultation needs.
  • Provide clinical consultation and collaborate with behavioral health staff to support appropriate care and professional growth.
  • Provide recurring leadership and support to behavioral health team leads at two assigned family medicine clinics.
  • Maintain cross-coverage capacity with other Behavioral Health Leadership Team members to ensure continuity of operations.
  • Teach medical residents, fellows, Master of Social Work students, and care team members about behavioral health issues, evidence-based interventions, primary care integration, and patient engagement and communication strategies.
Desired Qualifications
  • Experience as a Behavioral Health Consultant in a primary care setting.
  • Experience supervising graduate students in an educational setting.
  • Understanding of Federally Qualified Health Center grants and reporting guidelines.
  • Interest and experience in program development, improvement, and assessment.
  • Additional language ability in Spanish, Vietnamese, Mandarin, Cantonese, Russian, Arabic, Korean, or American Sign Language.
