Requirements
  • A bachelor's degree is preferred, or a high school diploma and at least two years of equivalent work experience is required.
  • Two years of experience in healthcare, healthcare-related case management, or court liaison duties, or one year of related experience in a psychiatric healthcare facility, is required.
  • Knowledge of patient rights, the Emergency Medical Treatment and Labor Act, mental health laws, psychiatric nursing regulations, and treatment of mentally and emotionally disturbed patients is required.
  • Knowledge of treatment procedures, interventions for acute psychosis, nonviolent crisis intervention practices, medical and psychiatric emergency procedures, and crisis intervention is required.
  • Ability to exercise judgment when answering questions and releasing information in accordance with HIPAA and Personal Health Information Act requirements is required.
  • Ability to document case management and discharge plans clearly and concisely is required.
  • Knowledge of state and federal programs and regulations governing hospital discharge planning, medical care, financial support, and community resources is required.
  • Ability to manage multiple tasks, prioritize work, and use strong time-management skills is required.
  • Ability to communicate effectively in English and exchange information with diverse individuals is required.
  • Ability to handle confidential information sensitively is required.
Responsibilities
  • Conduct thorough assessments of patients' mental health status, treatment history, and support systems to identify their needs and challenges.
  • Collaborate with multidisciplinary teams, including psychiatrists, psychologists, social workers, and nurses, to gather relevant information and insights.
  • Create individualized discharge plans with patients, caregivers, and healthcare providers, incorporating treatment-team input and patients' preferences and resources.
  • Identify and address barriers to successful discharge, including housing instability, lack of social support, and financial constraints.
  • Coordinate with community resources such as outpatient mental health clinics, housing assistance programs, vocational rehabilitation services, and peer support groups to establish post-discharge support networks.
  • Maintain accurate and current documentation of patient assessments, discharge plans, progress notes, and referrals.
  • Monitor patients' progress and adherence to discharge plans through follow-up contacts, phone calls, and appointments, adjusting plans as circumstances or treatment goals change.
  • Coordinate communication with healthcare providers, social service agencies, community organizations, and insurance companies to ensure continuity of care and smooth patient transitions.
  • Participate in interdisciplinary treatment-team meetings concerning patient progress, treatment recommendations, and discharge-planning strategies.
  • Provide rehabilitative services and initiate crisis intervention when needed.
  • Ensure follow-up appointments, safety plans, transportation, living arrangements, and needed medications are arranged after discharge.
Desired Qualifications
  • Direct practice in mental health treatment and experience in crisis intervention and customer service are preferred.
  • Bilingual English/Spanish communication is preferred but not required.
  • Cultural sensitivity and demonstrated ability to work with diverse people groups are preferred.
