At least 3 years of nursing experience is required, including 1 year of full-time direct patient care experience in hospice, home health, or oncology.

Demonstrated ability to manage all aspects of terminal care, including crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.
Demonstrated ability to provide a supportive and sensitive approach to staff and patient or family needs.
Computer proficiency in typing and various programs, including experience with electronic medical records.
Current CPR certification must be maintained if required by the state.
A current driver's license, vehicle insurance, and access to dependable transportation are required.
Responsibilities
Coordinate and oversee a spectrum of patient services in alignment with established hospice program policies.
Provide guidance and support to a team of clinicians to continuously improve hospice care services.
Conduct chart audits and oversee medical record maintenance for compliance with regulations, certification standards, and ethical care benchmarks.
Participate in developing orientation and in-service training for nursing and clinical staff.
Evaluate and improve clinical staff performance during probationary periods and annually, providing teaching and counseling to enhance service delivery.
Lead interdisciplinary team meetings to refine patient and family care plans, exchange insights, resolve challenges, and support professional collaboration.

Responsibilities

Qualifications

Demonstrated ability to manage all aspects of terminal care with expertise in crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.
Demonstrated ability to execute a supportive, sensitive approach to staff and patient/family needs.
Must be computer proficient in typing and various programs, including background in EMR.
Possess and maintains current CPR certification if required by state.
Current driver's license, vehicle insurance, and access to dependable transportation.
$100,000 - $120,000 per year (Average Salary Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
