Post-acute care SaaS for SNF operations
Prysm Health provides a SaaS platform for post-acute care, focusing on skilled nursing facilities in the United States. The product unifies clinical and administrative workflow using Unified Documentation, validates data at entry with Compliance at Entry, and includes MDS and PDPM modules plus an Operator Command Center that shows real-time census, staffing, and task status with automatic escalation. It differentiates itself through a domain-experienced team and by replacing fragmented, outdated software with a single integrated system that gives portfolio-wide visibility. The company aims to streamline facility operations, ensure regulatory compliance, and improve patient care and reimbursement across its client facilities.
