  • A valid and active Registered Nurse license in the state of practice is required.
  • Willingness to travel to patients' homes is required.

Responsibilities
  • Conduct thorough patient assessments and provide personalized, comprehensive care.
  • Educate patients and their families about health maintenance and disease prevention.
  • Assist with Activities of Daily Living as needed.
  • Coordinate schedules directly with patients according to their plan of care, approved hours, and preferences.
  • Monitor and promptly report changes in patient symptoms or behavior.
  • Administer medications and treatments as prescribed by the treating physician.
  • Communicate with case managers and collaborating physicians or specialists to coordinate patient care.
  • Maintain accurate patient logs and documentation in the electronic medical record system.
  • Provide advice and emotional support to patients and their family members.
  • Prepare and provide clear clinical notes and charting.
