Licensed Clinical Social Worker
About the job
- An LCSW credential is required.
- Two to four years of social work experience within the last five years are required; MSW fieldwork does not count.
- Provide telephonic psychosocial assessments and interventions to assist patients and caregivers with care transitions and coping with illness, diagnosis, treatment, or life situations, including emotional, mental, and substance abuse disorders.
- Coordinate appropriate services, including home health or hospice care and other levels of care such as skilled nursing facilities, board and care, and in-home caregivers.
- Educate patients, family members, and caregivers about adapting to the patient’s diagnosis, illness, treatment, or life situation.
- Participate in multidisciplinary healthcare teams.
- Assist with advance care planning and completion of advance directives and Physician Orders for Life-Sustaining Treatment (POLST).
- Coordinate symptom management and end-of-life services.
- Assess and refer patients for Medi-Cal or other insurance eligibility based on new entitlements, and refer for Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI) as needed or within scope of care definitions.
- Experience in a managed care environment is preferred.
