Requirements
  • Hold a valid, unencumbered license or verification to practice as a licensed practical or vocational nurse in the state where services are provided.
  • Have reliable transportation.
  • Provide current tuberculosis test results or a chest X-ray.
  • Provide current cardiopulmonary resuscitation certification according to agency policy.
  • Be flexible and organized, and exercise independent, sound judgment to perform the job duties and responsibilities.
  • Be able to perform the required physical activities for safe client care, including sitting, standing, stopping, bending, kneeling, walking, balancing, crouching, crawling, pushing, pulling, and reaching.
  • Have vision and hearing capabilities within normal limits, with or without aid.
  • Be able to frequently lift or move up to 15 pounds and occasionally lift or move 100 pounds or more when applicable.
  • Be able to safely provide the anticipated level of physical assistance required for client care.
Responsibilities
  • Deliver patient care to assigned patients using the nursing processes of assessment, planning, implementation, and evaluation in accordance with applicable laws, standards, policies, procedures, guidelines, and regulations.
  • Provide skilled nursing as ordered in the plan of treatment and nursing care plan.
  • Provide skilled nursing care during each scheduled visit in accordance with physician orders.
  • Identify and follow through on new problems, changes, and resolutions by notifying the director or supervisor, obtaining physician orders when needed, and documenting actions in clinical notes, supplemental physician orders, and the nursing care plan.
  • Observe aseptic technique as required.
  • Perform specific treatments and administer medications according to the care plan.
  • Work as a team member under the direction of the registered nurse.
  • Communicate skilled observations and significant changes in patient status to the Director of Clinical Services or Clinical Manager.
  • Request order modifications or additional orders based on the patient's clinical status and nursing judgment.
  • Document physician collaboration in the medical record.
  • Demonstrate and document appropriate use of available services.
  • Participate in agency-mandated activities, such as in-services, as requested by the agency director.
  • Complete clinical documentation accurately and submit it on time according to agency policy and guidelines.
  • Maintain a professional appearance and adhere to the dress code.
  • Report on time and as scheduled to complete work within designated hours.
  • Travel frequently to private homes to complete home health visits.
  • Notify the supervisor immediately when the required level of patient assistance exceeds what can be safely provided.
