Requirements
  • A Master of Social Work degree.
  • A state-required social work license.
  • Two to five years of related experience.
  • Willingness to work at both Avantus Harlem Green and Upper Manhattan Dialysis Center.
  • Ability to travel when working across multiple facilities.
Responsibilities
  • Provide psychosocial services to in-center and home dialysis patients, as applicable, using accepted social work practice methods.
  • Work with the health care team to promote positive adjustment, rehabilitation, and improved quality of life for patients.
  • Educate and support staff regarding the emotional, psychological, and behavioral impact of chronic kidney disease on patients and families.
  • Participate in quality indicators, quality assessment and improvement activities, patient satisfaction initiatives, and process improvement activities.
  • Assess patients’ psychosocial status, strengths, and needs as part of comprehensive patient assessment.
  • Participate in care planning with patients and the healthcare team to identify interventions supporting rehabilitation, treatment goals, and quality of life.
  • Use patient education programs, social work theory and methods, social work interventions, and quality-of-life measurement instruments in assessment and care planning.
  • Monitor patients and provide interventions to support adjustment to dialysis and optimal psychosocial status and quality of life.
  • Provide supportive counseling within the scope of clinical training and state licensure.
  • Provide educational and goal-directed counseling to patients seeking transplantation.
  • Refer patients to community resources, including home health services and vocational rehabilitation.
  • Maintain knowledge of local vocational and educational rehabilitation programs and assist patients with referrals and access.
  • Assess awareness of advance directives, provide access to related information, and facilitate discussions with patients, families, and the healthcare team.
  • Participate with the physician and nurse in discussions of patient do-not-resuscitate status to support informed decisions.
  • Follow social work and patient physical and mental function policies, including documentation requirements.
  • Document social work interactions and interventions provided to patients and families.
  • Provide psychosocial support and social-work interventions addressing non-adherence, quality outcomes, and quality-of-life concerns.
  • Participate in monthly quality review meetings and report quality indicators related to adherence, treatment duration, quality-of-life trends, and service recovery.
  • Assess patients’ knowledge of kidney disease and address barriers affecting treatment adherence.
  • Provide education tailored to patients’ learning styles, communication barriers, and needs.
  • Provide information about available treatment modalities with the interdisciplinary team.
  • Facilitate the transplant referral process and collaborate on transplant waitlist management.
  • Educate patients and families about psychosocial issues related to end-stage renal disease and available support services.
  • Review patient rights, responsibilities, grievance information, and facility policies with patients or their representatives.
  • Review admission-related rights, responsibilities, grievance procedures, nondiscrimination policies, do-not-resuscitate statements when applicable, and immediate needs or concerns.
  • Support the referral and admission process, including scheduling needs that maximize adherence and adjustment.
  • Interview patients requesting discharge to identify root causes or concerns and share them with operational leadership.
  • Liaise with insurance coordinators regarding patient insurance issues.
  • Educate patients about dialysis payment sources, including federal, state, commercial, renal-program, AKF HIPP, and entitlement programs.
  • Report patient status changes affecting insurance, including transplantation, discharge, loss of coverage, or extended travel.
  • Refer patients to the patient billing solutions department for treatment-bill questions and concerns.
  • Work with administrative support staff to maintain current patient resource lists, including transportation resources.
  • Provide training to staff on psychosocial topics as needed.
  • Participate in weekly team huddles and discuss urgent patient issues with staff.
  • Maintain a caseload in accordance with state regulatory guidelines.
  • Perform other related duties as assigned.
