Requirements
  • Current professional licensure as a California Registered Nurse, Licensed Clinical Social Worker, Medical Doctor or Doctor of Osteopathic Medicine, Nurse Practitioner, or Physician Assistant.
  • Experience with Medi-Cal underserved and culturally diverse populations.
  • Audit and oversight experience.
  • Previous care management or care coordination experience.
  • Strong clinical acumen.
  • Ability to work effectively within a multidisciplinary team.
  • Ability to review patient assessments and care plans, cross-reference medical history, and recommend modifications to care management activities for optimal patient care.
  • Ability to provide coaching and feedback to multidisciplinary team staff.
  • Ability to recommend practical approaches using innovation, judgment, creativity, and strategic thinking.
  • Strong administrative and computer navigation skills using Microsoft Office products and electronic medical record systems.
  • Modern office practices and procedures, including email.
  • Strong verbal and written communication skills.
  • Strong problem-solving, planning, and organizational skills.
  • Attention to detail and follow-through on work tasks.
  • Ability to handle multiple tasks simultaneously.
Responsibilities
  • Conduct billable care coordination activities and case conferences at the frequency set by department leadership and contracted health plan requirements.
  • Document all billable Enhanced Care Management time in the electronic health record.
  • Bill for Enhanced Care Management care coordination time.
  • Provide training and clinical guidance to Enhanced Care Management staff on comprehensive assessments, care management planning, and care goals for enrolled patients.
  • Provide tools and support to Enhanced Care Management staff to meet compliance and productivity goals.
  • Support staff in managing complex and high-acuity cases by providing immediate clinical guidance as needed.
  • Review assessment results and inform the care team of the recommended care plan.
  • Sign off on comprehensive assessments and care plans within required timeframes and contracted health plan guidelines.
  • Serve as a clinical resource for the care team through the development and implementation of care plans.
  • Conduct patient follow-ups when clinical support extends beyond the scope of a Lead Care Manager or Community Health Worker.
  • Facilitate access to primary care and behavioral health providers as needed.
  • Coordinate and prepare for audits from health plans and the California Department of Health Care Services.
  • Conduct comprehensive case reviews and provide final approval for patients or cases that Lead Care Managers seek to graduate from the Enhanced Care Management program.
  • Meet productivity goals for documentation, time logging, and billing for enrolled patients.
  • Oversee the clinical component of the Enhanced Care Management program.
  • Monitor staff training to ensure clinical assessments and care plans pass health plan and California Department of Health Care Services audits.
  • Coordinate, schedule, and participate in multidisciplinary team meetings in accordance with health plan and California Department of Health Care Services requirements.
  • Maintain direct communication with the Population Health Director regarding clinical Enhanced Care Management activities.
  • Monitor documentation accuracy for assessments, follow-ups, care plans, and claims through regular staff audits and process reviews.
  • Attend and participate in required department, program, and employee meetings.
  • Attend workshops and seminars as necessary to increase skills and knowledge for effective care, treatment, and leadership.
  • Support the health center's needs by working flexible or extended hours when necessary.
  • Complete required continuing education hours each year.
  • Maintain patient and team-member confidentiality in accordance with organizational rules and general Health Insurance Portability and Accountability Act regulations.
  • Promote positive staff interaction and open communication with other programs and departments.
  • Perform other work-related duties assigned by the supervisor.
