About the job
- A nursing degree or diploma from an accredited college, university, or nursing school is required.
- An active registered nursing or licensed practical nursing license within the State of New Hampshire is required.
- Annual continuing education units must be maintained.
- Current CPR certification is required.
- Perform professional nursing duties in accordance with facility policies and procedures, standards of professional practice, the State of New Hampshire Nurse Practice Act, OBRA, and HIPAA.
- Collaborate with interdisciplinary team members to formulate and update individualized resident care plans and participate in resident care plan meetings.
- Implement care protocols for falls prevention, safety and elopement, behavior management, weight management, skin integrity, bowel and bladder management, infection control, and pain assessment and management.
- Assess and document changes in residents’ physical or mental status, report findings to the primary care physician and treatment team, and provide clinical data for decision-making.
- Provide leadership and direction to direct caregivers, prepare daily assignments, delegate workload, and supervise direct care through observation and evaluation of results.
- Establish positive working relationships with team members, residents, and family members to facilitate open communication.
- Obtain, administer, and document medications and treatments; inventory and maintain the locked narcotics system.
- Support, create, and maintain a positive culture for team members and residents.
- Experience in a long-term care facility is preferred.
- Specialized knowledge of nursing care for elderly residents and rehabilitative nursing is preferred.
- Certification in intravenous administration is preferred.
